The aim of this project proposal is to improve the quality of life and access to integrated services for social inclusion of people with disabilities and people over 65 years old, inability to self-service, by providing in a family environment a personal/social assistant and a home assistant, but also psychological, social and health services tailored to the individual needs of the target group. For the purpose of the project, a “Centre for the provision of integrated services in a family environment” will be established on the territory of Rakovski Municipality, whose employees will provide specialised social and health services. The project foresees that care should be provided within 18 months, with a total duration of care, including establishment of the centre and recruitment and training of staff, provision of integrated services and a final phase covering a total of 22 months. In the newly created centre on the territory of the municipality, services will be offered mainly in a home environment by 10 social and 10 personal assistants, 60 domestic workers, a social worker, a psychologist and a nurse. It is planned that the staff will take care of up to 82 disabled users and persons over 65 years old, and at least 60 of them will have improved access to services by the end of the project. Taking into account the integrated and innovative nature of the services in this project, individual needs assessments, including new ones of the targeted persons under the New Alternatives procedure, will initially be prepared. This activity is intended to be carried out by the specialised staff of the newly created centre, respecting the principles of equal opportunities and non-discrimination and gender equality. Improving the quality of services will be achieved by ensuring integrated access to health, social and psychological services, depending on the individual needs of target users. Целта на настоящото проектно предложение е да се подобри качеството на живот и достъпа до интегрирани услуги за социално включване на хора с увреждания и хора над 65г. в невъзможност за самообслужване, чрез предоставяне в семейна среда на услугите личен/социален асистент и домашен помощник, но също така и на психологически, социални и здравни услуги, съобразени с индивидуалните потребности на целевата група. За целта на проекта, на територията на Община Раковски ще бъде създаден "Център за предоставяне на интегрирани услуги в семейна среда", чиито служители ще доставят специализирани социални и здравни услуги. В проекта е предвидено грижите да се предоставят в рамките на 18 месеца, като цялата му продължителност, включително сформиране на центъра и подбор и обучение на персонала, предоставяне на интегрираните услуги и заключителен етап да обхванат общо 22 месеца. В новосъздадения на територията на общината център, ще се предлагат услуги, доставяни основно в домашна среда, от 10 социални и 10 лични асистенти, 60 домашни помощници, социален работник, психолог и медицинска сестра. Предвидено е персоналът да обгрижва до 82 потребителя-лица с увреждания и лица над 65г. в невъзможност за самообслужване, като към края на проекта най-малко 60 от тях да бъдат с подобрен достъп до услуги. Съобразно интегрирания и иновативен характер на услугите в този проект, първоначално ще бъдат изготвени индивидуални оценки на потребностите, включително и нови такива на лицата от целевата група на процедура "Нови алтернативи". Предвидено е тази дейност да бъде изпълнена от специализирания персонал на новосъздадения център като бъдат спазени принципите за равни възможности и недопускане на дискриминация и равенство между половете. Подобряване качеството на услугите ще бъде постигнато чрез осигуряване на интегриран достъп до здравни, социални и психологически услуги в зависимост от индивидуалните нужди на потребителите от целевите групи.
