The main objective of the project proposal and to build a model for hourly mobile integrated health and social services — patronage care for deprived persons with disabilities and elderly people in the municipality of Asenovgrad to improve the quality of life and their opportunities for social inclusion. Municipality of Asenovgrad will provide suitable premises for which repairs will be carried out and equipment and furniture will be purchased. A car will be purchased to ensure the mobility of the service. A team of specialists (nurses, domestic orderlies, social workers, etc.) will be recruited to provide various health and social support activities. Support for the staff directly involved in the provision of health and social services — trainings and supervision is envisaged. In the implementation of the activities will be covered and will provide access to long-term mobile and integrated health-social care, according to the specific needs of 168 persons from the target groups, namely: Elderly people over 65 years of age with restrictions or inability to self-service; People with disabilities and their families. They will also receive psychological support and counselling. Patronage care will be provided 12 months within the project, and after its completion the municipality will look for other funding opportunities.The project proposal is developed in accordance with Measure 2 (Operation 6): Provision of patronage care through the provision of hourly mobile integrated health and social services for deprived persons with disabilities and the elderly of the Action Plan 2018-2021 for the implementation of the National Strategy for Long-term Care, adopted by the Council of Ministers No 28/19.01.2018 and is part of the overall process of deinstitutionalisation of care for people with regulated and elderly people and building alternative support in the community through the provision of quality and affordable long-term care services.     Основната цел на проектното предложение и да се изгради модел за почасови мобилни интегрирани здравно-социални услуги – патронажна грижа за нуждаещи се лица с увреждания и възрастни хора в община Асеновград за подобряване качеството на живот и възможностите им за социално включване.Община Асеновград ще осигури подходящи помещения, на които ще бъде извършен ремонт и ще се закупи оборудване и обзавеждане. Ще се закупи автомобил за осигуряване на мобилността на услугата. Ще бъде нает екип от специалисти (медицински сестри, домашни санитари, социални работници и други), които ще предоставят различни здравни и социални дейности за подкрепа. Предвидена е подкрепа на персонала, ангажиран пряко с предоставянето на здравно-социалните услуги – обучения и супервизия.При реализирането на дейностите ще бъдат обхванати и ще се осигури достъп до дългосрочна мобилна и интегрирана здравно-социална грижа, според специфичните потребности на 168 лица от целевите групи, а именно: Възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване; Хора с увреждания и техните семейства. Те ще получат и психологическа подкрепа и консултиране. Патронажната грижа ще се предоставя 12 месеца в рамките на проекта, а след неговото приключване общината ще търси други възможности за финансиране.Проектното предложение е разработено в съответствие с Мярка 2 (операция 6): Осигуряване на патронажна грижа чрез предоставяне на почасови мобилни интегрирани здравно-социални услуги за нуждаещите се лица с увреждания и възрастни хора на Плана за действие за периода 2018 – 2021 г. за изпълнение на Националната стратегия за дългосрочна грижа, приет с РМС № 28/19.01.2018 г. и е част от цялостния процес на деинституционализация на грижата за хората с уреждания и възрастните хора и изграждането на алтернативна подкрепа в общността чрез предоставянето на качествени и достъпни услуги за дългосрочна грижа.
