    OBJETIVOS: La compulsividad subyace a diversos trastornos mentales crónicos y discapacitantes como el Trastorno Obsesivo-Compulsivo (TOC) y el Juego Patológico (JP), y conlleva la pérdida de control sobre un gran número de comportamientos. El objetivo de nuestro proyecto es identificar factores comunes y diferenciales (a nivel de neuroimagen, neurocognición, neuropsicología y neuroinflamación) en ambos trastornos, y evaluar los cambios asociados a la respuesta terapéutica. Los resultados ayudarían a validar un constructo de compulsividad que podría ser adaptado a las recomendaciones del National Institute of Health (NIH) relativa a los Research Domain Criteria (RDoC). METODOLOGÍA: DISEÑO: Estudio multicéntrico prospectivo de cohortes (medidas repetidas intra-paciente antes y después de la terapia habitual) y comparación entre grupos (TOC y JP). MUESTRA: 60 pacientes afectos de TOC y 60 pacientes afectos de JP según criterios DSM-IV-TR. VARIABLES: Se recogerán variables clínicas y socio-demográficas, psicométricas (gravedad de la clínica obsesiva y de la conducta de juego, personalidad, creencias disfuncionales), neuropsicológicas (para evaluar impulsividad motora, flexibilidad cognitiva, toma de decisiones, aprendizaje de hábitos y de actividades dirigidas a un fin, entrenamiento de la evitación), de neuroimagen (resting-state, stop task, set shifting task, monetary incentive task delay, espectroscopía) y de neuroinflamación (prostaglandinas pro-inflamatorias -PGE2- y anti-inflamatorias -15d-PGJ2-, marcadores de estrés oxidativo - niveles de NOx-, expresión proteica y síntesis (mRNA) de iNOS, COX2 y S100-A10, entre otros). ANÁLISIS: El análisis combinado de los datos obtenidos de las diferentes exploraciones facilitará la identificación de biomarcadores y otros correlatos subyacentes a la conducta compulsiva. (Spanish)

    OBJECTIVES: Compulsivity underlies various chronic and disabling mental disorders such as Obsessive-Compulsive Disorder (OCD) and Pathological Game (PG), and leads to loss of control over a large number of behaviors. The objective of our project is to identify common and differential factors (at the level of neuroimage, neurocognition, neuropsychology and neuroinflammation) in both disorders, and to evaluate the changes associated with the therapeutic response. The results would help validate a compulsive construct that could be adapted to the recommendations of the National Institute of Health (NIH) regarding Research Domain Criteria (RDoC). METHODOLOGY: DESIGN: Prospective multicenter cohort study (repeated intra-patient measures before and after regular therapy) and cross-group comparison (TOC and JP). SAMPLE: 60 patients affected by OCD and 60 patients with JP according to DSM-IV-TR criteria. VARIABLES: Clinical and socio-demographic, psychometric variables (gravity of obsessive clinic and play behavior, personality, dysfunctional beliefs), neuropsychological variables (to evaluate motor impulsivity, cognitive flexibility, decision making, learning habits and activities aimed at an end, training of avoidance), neuroimage (resting-state, stop task, set shifting task, monetary incentive task delay, spectroscopy) and neuroinflammation (pro-inflammatory prostaglandins -PGE2- and anti-inflammatory -15d-PGJ2-, markers of oxidative stress — NOx levels-, protein expression and synthesis (mRNA) of iNOS, COX2 and S100-A10, among others). ANALYSIS: The combined analysis of data obtained from different explorations will facilitate the identification of biomarkers and other correlates underlying compulsive behaviour. (English)
