Objetivos: Determinar la relación causal bidireccional que existe entre la disfunción vascular y la fragilidad y el impacto de ésta como predictor de demencia. Diseño: Estudio poblacional, longitudinal, tipo corte abierta, con cortes transversales cada tres años. Este proyecto corresponde al tercer corte a la cohorte. Sujetos del estudio: 2.721 sujetos mayores de 65 años componentes de la cohorte del estudio Toledo de Envejecimiento Saludable-ETES, con sobre-representación de nonagenarios y centenarios. Metodología: Variables de clasificación basal de los sujetos, todas ellas recogidas en 2011-2013: a) vasculares: onda del pulso, ADMA sérico, ITB, y la presencia de enfermedad cardiovascular clínica basal (claudicación intermitente, ACV, insuficiencia cardiaca, cardiopatía isquémica), b) Variables nutricionales y composición corporal: MNA, MEDAS y DEXA, c) Medidas de fragilidad: Rasgo de Fragilidad (ETES), Criterios de Fried (CHS), Indice de Fragilidad (Rockwood). Variables resultado, a recoger en 2016-2018: a) Fragilidad incidente, b) Demencia incidente, mediante un proceso de dos fases (un primero de cribado siguiendo criterios pre-establecidos y posteriormente una evaluación clínica presencial por un geriatra experto en demencia), c) Eventos vasculares clínicos y subclínicos incidentes. Objectives: Determine the bidirectional causal relationship between vascular dysfunction and fragility and its impact as a predictor of dementia. Design: Population study, longitudinal, open cut type, with cross-sections every three years. This project corresponds to the third cut to the cohort. Subjects of the study: 2,721 subjects older than 65 years of age, members of the Toledo study on Healthy Ageing-ETES, with overrepresentation of nonagenarians and centenarians. Methodology: Baseline classification variables of subjects, all collected in 2011-2013: vascular: pulse wave, serum ADMA, BTI, and the presence of baseline clinical cardiovascular disease (intermittent classification, stroke, heart failure, ischemic heart disease), b) Nutritional variations and body composition: MNA, MEDAS and DEXA, (c) Fragility measures: Fragility Trait (ETES), Fried Criteria (CHS), Fragility Index (Rockwood). Output variables, to be collected in 2016-2018: a) Incident fragility, b) Incident dementia, through a two-phase process (a first screening according to pre-established criteria and subsequently an in-person clinical evaluation by a geriatrician expert in dementia), c) Clinical vascular events and subclinical incidents.
