Projekt jest skierowany do 50 (30K,20M) ON, zamieszkujących gminy Chodzież, Chodzież - gmina wiejska, Margonin, Szamocin, Jastrowie, Okonek, Tarnówka - gmina wiejska, Łobżenica, Miasteczko Krajeńskie - gmina wiejska, Wyrzysk. Osoby te są jednocześnie zagrożone wykluczeniem społecznym z powodu ubóstwa. W ramach projektu objęci zostaną nastepujacymi formami wsparcia: Poradnictwo zawodowe (06.2016-05.2017) Pośrednictwo pracy (09.2016-08.2017) Aktywizacja społeczna (12.2016-08.2018) Szkolenia zawodowe (01.2017-03.2018) Staże zawodowe (04.2017-06.2018) Zatrudnienie wspomagane (06.2016-09.2018) Blok warsztatów (12.2016-09.2018) W efekcie 56% z45UP osiągnie wskaźnik efektywności społeczno-zatrudnieniowej, a 22% spośród nich uzyska zatrudnienie/samozatrudnienie. Natomiast 46% spośród 5 UP ze znacznym lub umiarkowanym stopniem niepełnosprawności osiągnie wskaźnik efektywności społeczno-zatrudnieniowej, a 12% z nich podejmie zatrudnienie/samozatrudnienie (Polish)
