L’action proposée vie à mettre en oeuvre un **accompagnement renforcé des personnes en situation de handicap bénéficiaires des minimas sociaux (RSA) et, par extension, des personnes fragilisées par leur état de santé**. Sa méthodologie consiste à proposer un accompagnement individualisé (référent de parcours), combiné avec des modules collectifs, utiles pour générer des dynamiques groupales visant la reprise de confiance en soi et l’engagement dans un nouveau projet professionnel. En mobilisant une équipe pluridisciplinaire, disposant d’une expérience sur tous types de handicaps (formatrices, expertises de spécialité), nous pensons pouvoir déployer un programme qualitatif, visant une coopération étroite avec le service Cohésion sociale du Département et les partenaires prescripteurs (services sociaux des TDS, Pole emploi). La méthodologie et les différents outils mobilisés s’attachent à deux objectifs principaux, structurant l’accompagnement renforcé : * Lever les freins par un travail sur l’estime de soi et la reprise de confiance en soi. * Accompagner l’émergence d’un projet professionnel adapté à la situation de santé et viser des mises en ½uvre concrètes : périodes d’immersion en milieu de travail, accès à la qualification, accès à l’emploi. The proposed action to implement ** enhanced support for persons with disabilities who are beneficiaries of the social minima (RSA) and, by extension, those affected by their state of health**. Its methodology consists of offering individualised support (path reference), combined with collective modules, useful to generate group dynamics aimed at regaining self-confidence and engaging in a new professional project. By mobilising a multidisciplinary team, with experience on all types of disabilities (trainers, specialist expertise), we believe we can deploy a qualitative programme, aiming at close cooperation with the Department’s Social Cohesion Department and the prescribing partners (social services of TDS, Pole emploi). The methodology and the various tools mobilised focus on two main objectives, structuring enhanced support: * Lift the brakes by working on self-esteem and regaining self-confidence. * Support the emergence of a professional project adapted to the health situation and aim for concrete implementations: periods of immersion in the workplace, access to qualifications, access to employment.
