Projekt zakłada realizację usług społecznych na rzecz osób zagrożonych wykluczeniem społecznym na terenie miasta Gliwice. Usługami zostaną objęte przede wszystkim rodziny zagrożone wielowymiarowym wykluczeniem społecznym. Usługami społecznymi zostanie objętych min. 120 osób (90K, 30M) w tym 10% będą stanowiły osoby z niepełnosprawnościami. Uruchomione usługi, świadczone będą również w miejscu zamieszkania rodziny. Mają na celu wsparcie procesu deinstytucjonalizacji poprzez zapobieganie umieszczaniu dzieci w pieczy zastępczej i zmniejszeniu ryzyka wykluczenia mieszkaniowego (umieszczaniu w placówkach dla bezdomnych). Jest komplementarny z interwencją w ramach EFRR - projekt "Mieszkania na start - Adaptacja lokali komunalnych gminy Gliwice na cele mieszkań socjalnych - etap I". (Polish)
