The objectives were to be attained in the form of: : To increase access to support for dependants, to facilitate the residence of 21 people so far, increase the number and quality of care services provided at home for 21 people, combat poverty and exclusion of older/dependent 21 people, by action: 1. Care services for older people/dependent people, due care and support in life, age, state of health or disability — without such services, 2. Proj. skierow jest do osób starszych/niesamodz. wymagających opieki i pomocy w podstawowych czynnościach życiowych ze wzgl. na wiek, stan zdrowia lub niepełnosprawnosć 21 os. (15 K, 6 M) z terenu Gminy Sędziszów Małopolski. Zakłada zwiększenie dostępności ww. osób, zagrożonych ubóstwem i wykluczeniem społ. do usług społecznych, poprzez zwiększenie liczby miejsc świadcz. usług, liczby osób objętych usługami, liczbę godz świadcz usług w zakresie opieki nad osobami niesamodzielnym w okresie realizacji projektu od 01.03.2018 do 31.10.2020 i jego trwałości. Cele założ. w proj. tj. : - zwiększ. dostępu do wsparcia osobom niesamodzielnym, ułatwiającego pozostanie w dotychczas. miejscu zamieszkania 21 osób, - zwiększenie liczby i jakości usług opiekuń. świadczonych w miejscu zamieszkania dla 21 osób, - przeciwdziałanie ubóstwu i wykluczeniu społ. osób starszych/niesamodzielnych 21 osób realizowane będą poprzez działania: 1. Usł. opiekuńcze u osób starszych/niesamodzielnych, wymag. opieki i pomocy w podst. czynnościach życiowych ze wzgl. na wiek, stan zdrowia lub niepełnosprawnosć - nieposiadających takich usług, 2. Pomoc finans. zas.i cel. i/lub zas. na posił. dla UP, zgodnie z ust. o pom. społ. Gł. wskaźn. rezultatu - l. wspartych w programie miejsc świadczenia usług społ. istn. po zakończeniu proj. - 4 - l. utworzonych w programie miejsc świadczenia usług asyst. i opiek. istn. po zakon. proj. - 21 - l.os., które dzięki usługom opiekuń. pozbyły się obaw przed osamotnieniem i uwierzyły w lepsze jutro - 21 Gł. wskaźn. produktu - l. os. objętych usługami opiek. - 21 - l. os., u których przeprowadzono wywiady środowisk. - 21 - l. zawartych kontr. trójstr. - 21 Proj. prowadz. będzie w poszanow. równych praw i równych szans, w tym także, równości płci oraz dostępności dla osób z niepełnospr.
