The project involves the implementation of a comprehensive health programme which includes the integration of geriatric care with other forms of health and social care. The support will be targeted at 60 people (35K and 25M) on account of old age, including people with disabilities living in single-person households in Opolskie Province. The project will put in place a de-institutionalised form of care by providing transport for participants and/or providing psychological support to the actual carers in the period 05.2017-10.2019. The project guarantees the health, rehabilitation and care services necessary to achieve the objectives of the project, which go beyond the guaranteed provision of health care (the guaranteed service could not be financed by the individual in question from public funds over the lifetime of the project), namely, mobility, psychiatry and speech therapy, and increased access to care, rehabilitation and support equipment through the provision of equipment, including training/counselling, in conjunction with learning and counselling concerning its use and psychological support and/or training for medical care. The project will be carried out in cooperation with outpatient departments in order to make more effective use of the potential of medical/equipment staff/equipment AOS T.POZ — KAMMED, orthopaedic at the public health facility CHIR-ORTOMED SC, Brzezeski Szpital- Brzezeskie Centrum Medyczne. Projekt zakłada realizację kompleksowego programu prozdrowotnego uwzględniającego integrację opieki geriatrycznej z innymi formami świadczenia zdrowotnymi i społecznymi. Wsparcie zostanie skierowane do 60 osób (35K i 25M) niesamodzielnych ze względu na podeszły wiek, w tym osób z niepełnosprawnościami żyjących w jednoosobowych gospodarstwach domowych z terenu woj. opolskiego. W ramach projektu zostaną wdrożone zdeinstytucjonalizowanej formy opieki poprzez zapewnienie transportu dla uczestników i/lub zapewnienia wsparcia psychologicznego opiekunów faktycznych, w okresie 05.2017-10.2019. Projekt gwarantuje usługi zdrowotne, rehabilitacyjne, pielęgnacyjne niezbędne do realizacji celów projektu, które wykraczają poza gwarantowane świadczenia opieki zdrowotnej ( gwarantowana usługa zdrowotna nie mogłaby zostać sfinansowana danej osobie ze środków publicznych w okresie trwania projektu) tj. rehabilitacja ruchowa, psychiatryczna i logopedyczna oraz zwiększenie dostępu do sprzętu pielęgnacyjnego, rehabilitacyjnego i wspomagającego poprzez tworzenie wypożyczalni sprzętu, w tym szkolenia/doradztwo w połączeniu z nauką ich obsługi i doradztwem w zakresie jego wykorzystania i wsparcie psychologiczne i/lub szkolenia dla opiekunów faktycznych w zakresie opieki medycznej. Projekt będzie realizowany przy współpracy z jednostkami ambulatoryjnej opieki specjalistycznej w celu efektywniejszego wykorzystania potencjału kadry medycznej/zasobów lokalowych/sprzętowych AOS tj.POZ – KAMMED, Poradnia Ortopedyczna - Niepubliczny Zakład Opieki Zdrowotnej CHIR-ORTO-MED S.C., Brzeski Szpital- Brzeskie Centrum Medyczne. Projekt zakłada komplementarność z narzędziami zaplanowanymi w ramach programu SSD, pakiet IV Złota jesień oraz z innymi projektami współfinansowanymi z UE. (Polish)
