The subject of the project: The ‘age-friendly and disabled shop’ of the Municipal Centre for Social Assistance in the Bank is to increase and develop assistant and care services. These services will be taken forward by the Municipal Social Assistance Centre. These will be people or families at risk of poverty or social exclusion. The project was preceded by a study by social workers. The analysis resulted in the diagnosis of the main problems affecting the dependants of the Municipal Social Assistance Centre in the Bank. The most important problems for the elderly and disabled living in the area of the municipality of Brzeg belong to loneliness and a sense of incapacity. They lead to progressive marginalisation and exclusion from the local community. At the same time, the vast majority of the dependants do not allow the possibility of spending their life in a foreign place — at the Social Assistance House or at the Health Care Department. The main objective of the project is to prevent this phenomenon, especially for the elderly, single and dependent people. Placing these persons in care in the form of a system of telemedicine will improve their psychological comfort and safety. These people will be able to stay in their family, local communities and will not have to be covered by institutional care, for example in hospitals, ZOL, and community support homes. In addition to tele-care (for all project participants), additional support will be provided to people in need of support for care services, specialist care services and people with disabilities. These people will also be able to live in their home, local and not have to be covered by institutional care, once they have received specialist support. (English)

    Przedmiotem projektu pt.: „Brzeg przyjazny dla seniorów i osób niepełnosprawnych” realizowanego przez Miejski Ośrodek Pomocy Społecznej w Brzegu jest zwiększenie i rozwój usług asystenckich i opiekuńczych. Usługami tymi zostaną objęci podopieczni Miejskiego Ośrodka Pomocy Społecznej. Będą to osoby lub rodziny zagrożone ubóstwem lub wykluczeniem społecznym. Projekt został poprzedzony analizą przeprowadzoną przez pracowników socjalnych. W wyniku analizy zdiagnozowano główne problemy dotykające podopiecznych Miejskiego Ośrodka Pomocy Społecznej w Brzegu. Do najważniejszych problemów ludzi starszych i niepełnosprawnych mieszkających na terenie Gminy Brzeg należy samotność i poczucie nieprzydatności. Prowadzą one do stopniowej marginalizacji i wykluczenia ze społeczności lokalnej. Jednocześnie zdecydowana większość podopiecznych nie dopuszcza możliwości spędzenia schyłku swojego życia w obcym miejscu – w Domu Pomocy Społecznej lub Zakładzie Opieki Zdrowotnej. Głównym celem przyświecającym realizacji projektu jest zapobieganie temu zjawisku, zwłaszcza wśród osób starszych, samotnych i niesamodzielnych. Objęcie tych osób opieką w formie systemu telelopieki poprawi ich komfort psychiczny i poczucie bezpieczeństwa. Osoby te będą mogły pozostać w swoich środowiskach rodzinnych, lokalnych i nie będą musiały być objęte instytucjonalną opieką np. w szpitalach, ZOL, domach pomocy społecznej. Oprócz teleopieki (świadczonej dla wszystkich uczestników projektu) dodatkowym wsparciem zostaną objęte osoby potrzebujące wsparcia w zakresie usług opiekuńczych, specjalistycznych usług opiekuńczych oraz osoby niepełnosprawne. Również te osoby, po otrzymaniu specjalistycznego wsparcia, będą mogły funkcjonować w swoich środowiskach domowych, lokalnych i nie będą musiały być objęte opieką instytucjonalną. (Polish)
