The project involves the setting up of a Senior Citizens’ Support Centre and the establishment of an Independent ON Independent Person, who will be able to benefit from the benefit of a person from the district of limanowski. The main objective of the project is to establish a comprehensive programme of support for Senora, its family and ON, with a specific focus on carers of older people with chronic diseases. The creation of an interdisciplinary system is aimed at alleviating carers, improving care skills, improving the quality of home care and ensuring that care is provided for as long as possible in the local environment, the place of residence, within the family. The centre will also ensure daily care and activation of older people. The aim of this offer is to improve the functioning of seniors and their families, including their carers. The aim of the facility is to mobilise older people and enable them to function in society. Stimulating active ageing and taking a number of actions for their development is the most important in tackling social exclusion. More than this, alleviating their carers, can help to enable them to work, regenerate the psychological and thus contribute to increased care capacity. The target group of the project is the ON, the elderly themselves and their families. The project takes the view that the Centre’s offer will be able to benefit at least 30 persons from the county in the range of services on the one and at least 10 of the elderly and their families from the municipality of Limanowa in terms of day to day care and animation. The main effects of the project will be the improvement of the organisation of senior care through improved access to information and services in this field, the retention of family carers across the border, the improvement of the quality of life and health of the elderly and of the carer, the postponement of the need for institutional care. Two buildings will be created: Information and communication points and the Daily House of Care and Activation of Elderly Persons (English) | |||

Projekt zakłada utworzenie Centrum Wsparcia Seniora oraz Opiekuna Nieformalnego ON Osoby Niesamodzielnej, z którego oferty będą mogły skorzystać osoby z powiatu limanowskiego. Głównym celem przedsięwzięcia jest stworzenie kompleksowego programu wsparcia Seniora, jego rodziny oraz ON, ze szczególnym nastawieniem na opiekunów osób starszych, borykających się z przewlekłymi chorobami. Stworzenie interdyscyplinarnego systemu ma na celu odciążenie opiekunów, poprawę kompetencji opiekuńczych, podniesienie jakości opieki domowej oraz zapewnienie tej opieki jak najdłużej w lokalnym środowisku, miejscu zamieszkania, w otoczeniu rodziny. Centrum zapewniać będzie również dzienną opiekę i aktywizację osób starszych. Celem tej oferty jest polepszenie funkcjonowania seniorów oraz ich rodzin, w tym ich opiekunów. Oferta placówki służyć ma aktywizacji seniorów i umożliwiać im funkcjonowanie społeczne. Stymulowanie osób starszych w kierunku aktywności i podejmowanie szeregu działań na rzecz ich rozwoju ma największe znaczenie w zakresie przeciwdziałania wykluczeniu społecznemu. Ponad to, odciążenie ich opiekunów, może pozwolić na ich uaktywnienie zawodowe, regenerację sił psychicznych, a tym samym przyczyni się do zwiększenia wydolności opiekuńczej. Grupą docelową projektu są ON, sami seniorzy jak i ich rodziny. W projekcie przyjmuje się, iż z oferty Centrum skorzystać będzie mogło min. 30 osób z powiatu z zakresu usług wspomagających ON i min. 10 osób starszych i ich rodziny z gminy Limanowa z zakresu dziennej opieki i aktywizacji. Głównymi efektami projektu będzie poprawa organizacji opieki senioralnej poprzez ułatwiony dostęp do informacji i usług z tego zakresu, zachowanie rodzinnych zasobów opiekuńczychniwelowanie obciążenia obowiązkami, poprawa jakości życia i zdrowia seniora i opiekuna, odroczenie w czasie konieczności opieki instytucjonalnej. Powstaną dwa budynki: Punt InformacyjnoKonsultacyjny oraz Dzienny Dom Opieki i Aktywizacji Osób Starszych Bliżej Siebie (Polish)
