The aim of the project is to increase the availability of social services in the District of Proszowicki by creating a day-care centre for 40Ps (21K, 19M) for caretakers, nursing and activation services, as well as supporting 40 caregivers/families through assistive services. The project is aimed at the inhabitants of the county of powder who, due to old age, a condition of health or disability requires care or support to be provided in connection with the inability to carry out, on their own, at least one of the everyday activities of the day and their carers. The project plans to include 40 people including women and men. The facility will offer care during the day, including meals, rehabilitation, cultural activities, education, occupational group therapy and individual support for mental health, psychotherapy and other elderly people. All activities will take place under the care of experienced professionals, social workers, occupational therapists, physiotherapists. In addition, persons will be supported to ensure that an optimal physical and mental state is maintained, kept independent and active in order to counter the deepening of the effects of ageing and/or disabilities. In addition, the project will support families/carers of older people through training, guidance, information, psychological support. (English)

    Celem projektu jest zwiększenie dostępności usług społecznych na terenie Powiatu Proszowickiego poprzez utworzenie dla 40UP (21K,19M) dziennego ośrodka opieki dla osób niesamodzielnych w Pieczonogach, zapewnienie im usług opiekuńczo-pielęgnacyjnych oraz aktywizująco-usprawniających jak również wsparcie 40 opiekunów faktycznych/rodzin poprzez usługi wspomagające. Projekt skierowany jest do mieszkańców powiatu proszowickiego, którzy ze względu na podeszły wiek, stan zdrowia lub niepełnosprawność wymagają opieki lub wsparcia w związku z niemożnością samodzielnego wykonywania co najmniej jednej podstawowej czynności dnia codziennego oraz ich opiekunów faktycznych. W ramach projektu planuje się objąć działaniami 40 osoby w tym Kobiety i Mężczyźni. Placówka będzie oferować opiekę w ciągu dnia, w tym posiłki, rehabilitację, zajęcia kulturalno - oświatowe, terapię zajęciową grupową i indywidualną dla podtrzymywania kondycji psychofizycznej, psychoterapię i wspólne z innymi osobami starszymi spędzanie wolnego czasu. Wszystkie zajęcia będą odbywać się pod opieką doświadczonych specjalistów - pracowników socjalnych, terapeutów zajęciowych, fizykoterapeutów. Ponadto Osoby otrzymają wsparcie zapewniające utrzymanie optymalnego stanu psychofizycznego, podtrzymania samodzielności i aktywności w celu przeciwdziałania pogłębienia się dysfunkcji związanych z procesem starzenia się i/ lub niepełnosprawności. Dodatkowo projekt zakłada wsparcie dla rodzin/opiekunów faktycznych seniorów poprzez szkolenia, poradnictwo, informowanie, wsparcie psychologiczne. (Polish)
