GŁÓWNY CEL PROJ. zakłada zwiększenie(w okresie 01.10.2016-30.06.2018) zdolności do zatrudnienia i aktywnego uczestnictwa w życiu społ.-zawodowym min.45os. z GRUPY DOCELOWEJ 50os.(28K/22M) zagrożonych ubóstwem lub wykluczeniem społ.,w tym w szczególności min.28os. doświadczających wielokrotnego wykluczenia społ.(16K/12M), min.8os. korzystających z PO PŻ(4K/4M) i min.14 ON(8K/6M),w tym 12os. o znacznym lub umiarkowanym stopniu niepełnosprawności, os. z niepełnosprawnościami sprzężonymi, os. z niepełnosprawnością intelektualną lub os. z zaburzeniami psychicznymi(7K/5M), zamieszkujących w woj.małopolskim w powiatach: brzeskim, dąbrowskim, tarnowskim i m.Tarnów, zagrożonych skumulowanymi problemami społ. Osiągnięcie ww.celu umożliwi realizacja(w ramach 6 ZADAŃ MERYTORYCZNYCH) szerokiego wachlarza zindywidualizowanych działań aktywizacyjnych, obejmujących obligatoryjnie–identyfikację potrzeb i potencjału UP i dodatkowo co najmniej 3 usługi aktywnej integracji (udział zg. z wyznaczoną IŚR), w tym: poradnictwo psychospołeczne, psychologiczne i zawodowe, treningi umiejętności i kompetencji społ., szkolenia/kursy umożliwiające zdobywanie kwalifikacji, staże i pośrednictwo pracy, oraz uzupełniająco do ww.–specjalistycznie wsparcie o charakterze zdrowotnym. GŁÓWNE REZULTATY PROJ.: - min.16os. uzyska kwalifikacje po opuszczeniu programu - min.18os. będzie poszukiwać pracy - min.10os. podejmie zatrudnienie(w tym na własny rachunek) w okresie do 4tyg. po zakończeniu udziału w proj. -w okresie do 3m-cy po zakończeniu udziału w proj. postępu w aktywizacji/dalszej aktywizacji dokona *min.56%UP, a min.22%UP podejmie zatrudnienie *min.46% os. o znacznym stopniu niepełnosprawności, os. z niepełnosprawnością intelektualną i os. z niepełnosprawnościami sprzężonymi, a min.12% os. z ww.grup podejmie zatrudnienie (Polish)
