Projekt obejmuje:utworzenie nowego miejsca opieki medycznej w powiecie brzeskim poprzez rozwój opieki długoterminowej świadczonej na rzecz do15 osób niesamodzielnych z powiatu brzeskiego równocześnie w formie Dziennego Domu Opieki Medycznej dalej DDOM oraz wsparcie jego działalności razem 120 osób niesamodzielnych;Wsparcie dla 120 opiekunów, w szczególności członków rodzin szkolenia w zakresie opieki medycznej nad osobami niesamodzielnymi.Wsparcie będzie zgodne z dokumentem Ogólnoeuropejskie wytyczne dotyczące przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności oraz przyczyni się do wprowadzeniazwiększenia usług świadczonych na poziomie lokalnej społeczności.Usługi będą świadczone na poziomie lokalnej społeczności oraz w sposób: umożliwiający odbiorcom tych usług kontrolę nad swoim życiem i naddecyzjami, które ich dotyczą, zapewniający, że odbiorcy usług nie są odizolowani od ogółu społeczności lub nie są zmuszeni do mieszkania razem, gwarantujący, że wymagania organizacyjne nie mają pierwszeństwa przed indywidualnymi potrzebami mieszkańców.
