The project will support people at risk of poverty and social exclusion, in particular children and young people living in the tables (below the threshold for disadvantage) in Mazma.This project will be carried out exclusively in rural areas. There are no day-care facilities for children and young children. the project is set up in the form of a community centre and the applicant ensures that there are two key competences in the daily support facility, according to the identified needs of the participants, i.e. communication in foreign languages and learning to learn. Support will be provided for 24. — children and young people and 48 people from the project’s entourage, through the implementation of solutions developed in the framework of the innovation of POKL; The applicant shall ensure that support to children and young people from family experiencing difficulty in performing their functions will be carried out on the basis of an individual reintegration path, after a prior diagnosis of the needs and potential of the participants. Support will also be given to those who have an environment in the form of targeted family counselling to families facing difficulties in fulfilling their care and educational function: training of parental, social and daily competences. in the establishment of the children, they will spend their free time, their homework with the care of the educators and the use of specialist activities. they will be provided with a warm form. within the frame of the project, a TV, computer, armchairs, cupboards to equip the day-care centres will be purchased. The applicant shall ensure that the project support is undertaken in accordance with the ‘Pan-European guidelines for the transition from institutional to community based care’ and the document ‘Use of European Union funds for the transition from institutional to community-based care — Toolbox’.     Projekt obejmie wsparciem osoby zagrożone ubóstwem i wykluczeniem społecznym, w szczególności dzieci i młodzież zamieszkujące gm. Stoczek (poniżej progu defaworyzacji) w woj. maz.Projekt będzie realizowany wyłącznie na obszarze wiejskim. W gm. Stoczek nie ma placówek wsparcia dziennego dla dzieci i młodzieży.Założeniem projektu jest stworzenie placówki w formie świetlicy socjoterapeutycznej.Wnioskodawca zapewnia, że w placówce wsparcia dziennego będą realizowane zajęcia rozwijające dwie kompetencje kluczowe, zgodnie ze zdiagnozowanymi potrzebami uczestników, tj. porozumiewanie się w językach obcych i umiejętność uczenia się. Wsparciem zostanie objętych 24 os. - dzieci i młodzież oraz 48 osób z otoczenia uczestników projektu, poprzez wdrożenie rozwiązań wyprac. w ramach proj. innowac.POKL. Wnioskodawca zapewnia, że wsparcie dzieci i młodzieży z rodzin przeżywających trudności w wypełnianiu funkcji opiek.-wych. będzie realizowane na podst. indywidualnej ścieżki reintegracji, po uprzedniej diagnozie potrzeb i potencjału uczestników. Wsparcie otrzymają również osoby z otoczenia w postaci spec. poradnictwa rodzinnego dla rodzin przeżywających trudności w wypełnianiu funkcji opiekuńczo - wychowawczych w postaci zorganizowania zajęć: treningu kompetencji rodzicielskich, społecznych i życia codziennego.W placówce dzieci będą spędzały czas wolny, odrabiały lekcje pod okiem wychowawcy a także korzystały z zajęć specjalistycznych.Podopieczni będą mieli zapewniony ciepły posiłek.W ramach projektu zostaną zakupione telewizor, komputer, fotele, szafy w celu wyposażenia świetlicy. Wnioskodawca zapewnia, że wsparcie w projekcie odbywać się będzie zgodnie z "Ogólnoeuropejskimi wytycznymi dotyczącymi przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności" oraz dokumentem "Wykorzystanie funduszy Unii Europejskiej w celu przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności - zestaw narzędzi".
