It is clear that there is a need for reforms in the areas of social exclusion and health and is also justified by objective reasons. The main objective of the project is to support the deinstitutionalisation of dependent persons in the development of health services for dependent persons. The aim of the planned activities is to improve the functioning of the health system for care of dependent persons in the form of the implementation of organisational arrangements for the development of forms of health care. The project meets the challenges posed to the policy of a senior-level aimed, inter alia, at extending working lives and exploiting the potential of older people. The specific objectives are, inter alia, to improve the financial management of healthcare in the field of non-hospital medical care, to develop the medical services sector in close home environment, to improve the health of dependent persons, to develop new forms of day care for dependent persons and to reduce the number and duration of medically necessary hospitalisation. The DDOM will be isolated within the structure of the medicinal product (the attribution of the department code) until the date of signature of the contract and the adjustment shall be concentrated in the EG on the basis of a structural DDOM. The project is in line with the “Framework for long-term permanent policy in Poland for the period 2014-2020”, for the management of geriatric care”, ROP MP 2014-2020, national legislation, including PHP, with rules on state aid, the principle of equal opportunities for men and women, equal opportunities and non-discrimination, including accessibility for people with disabilities and the principle of sustainable development. The project is in line with the guidelines on the transition from institutional to community-based care and the implementation guidelines for ESF projects. (English)

    NIEPUBLICZNY ZAKŁAD OPIEKI ZDROWOTNEJ - "MÓJ LEKARZ" IZABELA KOŚCIUCZYK

    Oczywistym jest, że istnieje potrzeba dokonywania reform w obszarach wykluczenia społecznego oraz zdrowia jest ona dodatkowo uzasadnionea obiektywnymi przesłankami. Głównym celem projektu jest wsparcie deinstytucjonalizacji opieki nad osobami zależnymi w formie rozwoju świadczeń zdrowotnych dla osób niesamodzielnych. Zaplanowane działania mają na celu poprawę jakości funkcjonowania systemu ochrony zdrowia w zakresie opieki nad osobami niesamodzielnymi w formie wdrożenia rozwiązań organizacyjnych umożliwiających rozwój form świadczeń zdrowotnych. Projekt spełnia wyzwania postawione dla polityki senioralnej mającej na celu m.in. wydłużenie okresu aktywności zawodowej oraz wykorzystanie potencjału osób starszych. Celami szczegółowymi jest m.in. poprawa jakości zarządzania finansowego w ochronie zdrowia w zakresie medycznej opieki pozaszpitalnej, rozwój sektora usług medycznych świadczonych w środowisku zbliżonym do domowego, poprawa stanu zdrowia osób niesamodzielnych, rozwoju nowych form dziennej opieki nad osobami niesamodzielnymi oraz zmniejszenie liczby i czasu trwania nieuzasadnionych medycznie hospitalizacji. DDOM zostanie wyodrębniony w strukturze podmiotu leczniczego (nadanie kodu resortowego) do dnia podpisania umowy ORAZ ZOSTANIE ZATW REG ORG JEDN W KTOREJ ZOSTANIE WYODR STRUKTURALNIE DDOM. Projekt jest zgodny z „Założeniami długofalowej polityki senioralnej w Polsce na lata 2014-2020”, Standardami postępowania w opiece geriatrycznej”, RPO WM 2014-2020, prawodawstwem krajowym, w tym PZP, z zasadami dotyczącymi pomocy publicznej, zasadą równości szans kobiet i mężczyzn, zasadą równości szans i niedyskryminacji w tym dostępności dla osób z niepełnosprawnościami oraz zasadą zrównoważonego rozwoju. Projekt jest zgodny z wytycznymi dot przejścia z opieki instytucjonalnej do opieki świadczonej na poziomie społeczności lokalnych oraz wytycznymi w zakresie realizacji przedsięwzięć z udziałem EFS.
