The main objective of the project is to increase the availability of social and health services among the 60 inhabitants of the Municipality of Puławy in the period to 31.12.2021. In relation to older people, the project includes activities aimed at preventing disease and loss of efficiency and restoring independence. The project provides for facilitated access to health services only to the extent not covered by the Act of 27.08.2004 on healthcare services financed from public funds (Journal of Laws of 2015. (581) Tasks: 1.Creation of the 2.Operation of the Daily Service for the elderly, dependent and disabled.The working of the Daily House of Assistance for the 15 elderly, dependent and disabled 3.Establishment of a Senior Citizens’ Club — the environmental protection institution activating the elderly, independent and handicapped 4.Development and dissemination of forms of support in the place of residence for the 5.Creation of the.Establishment of LOCAL CENTRUM for senior citizens in the comprehensive action plan for the development of the family support system will help to improve the quality of social services and reduce one of the significant barriers to exclusion of people who are at risk of poverty or exclusion from access to essential public services. The project shall ensure that the local community is included in the actions carried out in accordance with the empowerment concept. The activities planned in the project will be implemented in cooperation with the social RAIA SUPPORT, which will participate in each phase of the project (diagnosis of needs, outreach to the potential mentees, support in the form of volunteering activities, monitoring of the performance of the presumed activity). Celem głównym realizowanego projektu jest wzrost dostępności usług społecznych i zdrowotnych wśród 60 mieszkańców Gminy Puławy w okresie do 31.12.2021 r. Projekt zakłada usługi świadczone w lokalnej społeczności (KRYT.SPEC.4) Projekt przewiduje realizację działań profilaktycznych odnoszących się do różnych usług wsparcia dla osób i ich rodzin, które mają zapobiec potrzebie umieszczenia w zakładzie. W stosunku do osób starszych projekt przewiduje działania nakierowane na zapobieganie chorobom i utracie sprawności oraz odzyskaniu niezależności. Projekt zakłada ułatwiony dostęp do usług zdrowotnych jedynie w zakresie nieobjętym przepisami ustawy z dnia 27.08.2004 r. o świadczeniach opieki zdrowotnej finansowanych ze środków publicznych (Dz.U. z 2015 r. Poz. 581) Zadania: 1.Utworzenie Dziennego Domu Pomocy dla osób starszych, niesamodzielnych i niepełnosprawnych 2.Funkcjonowanie Dziennego Domu Pomocy dla 15 osób starszych, niesamodzielnych i niepełnosprawnych 3.Utworzenie Klubu Aktywnego Seniora - środowiskowej instytucji aktywizującej osoby starsze, niesamodzielne i niepełnosprawne 4.Rozwój i upowszechnienie form wsparcia środowiskowego w miejscu zamieszkania 5.Utworzenie LOKALNEGO CENTRUM TELEOPIEKI DLA SENIORÓW Zaplanowane w projekcie kompleksowe działania na rzecz rozwoju systemu wsparcia rodziny przyczynią się do podniesienia jakości usług społecznych oraz ograniczenia jednej z istotnych barier jaką jest wykluczenie z dostępu do podstawowych usług publicznych warunkujących możliwości rozwojowe osobom wykluczonym, bądź zagrożonym ubóstwem i wykluczeniem. Projekt zapewnia włączenie społeczności lokalnej w realizowane działania zgodnie z koncepcją empowerment. Zaplanowane w projekcie działania realizowane będą we współpracy ze SPOŁECZNĄ RADA WSPARCIA, która uczestniczyć będzie w każdym etapie realizacji projektu (diagnozie potrzeb, dotarciu do potencjalnych podopiecznych, realizacji wsparcia w formie wolontariatu, monitorowaniu stopnia osiągniecia zakładanyc (Polish)
