Objective: by the end of December 2021, ensure a minimum of 30 inhabitants of the gminy of Cyców (including 20 women and 10 men) who are persons excluded or at risk of social exclusion (minimum 30 elderly people, including persons with disabilities) of high social quality, and supporting and activating community-based services for elderly and disabled people and accommodation in the form of disaggregated support and community-based care. The project foresees the implementation of preventive actions to prevent the need for an establishment. In relation to older people, the project provides for action on disease prevention and loss of efficiency and on recovery of independence, including the creation and operation of a support centre — the Seniors Seniors Centre, the main results of: L. supported under the social service provision existing at the end of the project — 40 (30 in the support centre + 10 in the residential units of the rental equipment. (2) Target: min. 30 people, residents of Cyclists, at risk of poverty or social exclusion of the target group; lime: 30 people in Age: older persons 60 +, including persons with disabilities, beneficiaries of the POSAMS and the social unit Place of residence: gm.Cycles, countryside professional status: unpaid professional (pensioners) financial status: income at the level of social assistance or family support, due to the need to support older people, and according to the gender breakdown of this age group, the target group is 20 women and 10 men (66 %: 33 %) (English) | |||

Cel: zapewnienie do końca grudnia 2021 roku minimum 30 mieszkańcom gminy Cyców (w tym 20 kobietom i 10 mężczyznom), będącymi osobami wykluczonym lub zagrożonymi wykluczeniem społecznym (minimum 30 osobom starszym, w tym osobom z niepełnosprawnościami) wysokiej jakości społecznych oraz wspierających i aktywizujących usług świadczonych w środowiskowym ośrodku wsparcia dla osób starszych i niepełnosprawnych oraz w miejscach zamieszkania w formie wsparcia zdeinstytucjonalizowanego i opieki środowiskowej.
