Our operation takes place in 3 territories: Vénissieux, Vaulx-En- Velin,Rhône Sud and is therefore the subject of 3 actions The principle of voluntary membership/engagement of the person in a reinforced and personalised socio-professional integration pathway is indispensable before any action is taken. Starting with the participant’s membership, a work is undertaken with him by the single path reference to establish in the first instance a detailed social, personal and professional diagnosis in order to assess his ability to adhere to enhanced support, to highlight his motivations, his assets and the problems identified as well as the objectives to be set in order to implement his project. Once its PLIE entry has been validated in CSP, we will draw up a realistic and progressive action plan with the participant in view of his overall situation and lead the participant to become an actor in his or her career. The path reference, resource person co-builds and implements in the framework of regular meetings with the participant the path relevant to the objectives targeted in the diagnosis, and ensures that the action is going smoothly. The referent is the guarantor of the support, the mobilisation of the actions/steps and the regular evaluation of these carried out with the stage referent.The referent of parours will favour multidisciplinary and shared diagnoses. It will report on the participant’s path in the tools defined by the PLIE To set up the action plan defined jointly, the path reference will enable the participant to be an actor in his/her path, in particular by direct access to information on actions, opportunities, support schemes offered by the PLIE (training, mobility, childcare, etc.) or other partners and by participating in the implementation of the PLIE. It will mobilise its networks, set up steps and actions to solve the problems identified by the participant. The referent will support the participant’s efforts by providing personalised, evolutionary support according to the reality of the needs. It will mark, secure as much as possible the course of the participant (support during the stage, preparation at the exit of the stage) by regular follow-up at both stage and job level. It participates in the dynamics of the network (employment, training, activity of integration through the economy, networking with the RECs and the fight against discrimination). It should be noted and it is very important given the typology of the public followed that our support is not limited in time which allows the action plan and the objectives set to be adjusted according to the progress of the participant’s integration pathway. During the first 6 months of taking up his duties, the referent will assist the participant in a job under ordinary law, setting up a company, and validating a qualifying training course. In summary, the accompaniment is as follows: 1- Phase Hospital Diagnostic Knowledge with the participant, verification of its elegibility, presentation of the framework and purpose of the follow-up and voluntary membership of the participant In-depth diagnosis shared on the basis of the CV and the experience of the person who will be formalised. Overall assessment of the situation that will allow us to intent the professional project. Formalisation of the first recommendations of the action plan in the contract of commitment attached to the PLIE presentation dossier and which will be the subject of a presentation in CSP together with the questionnaire collecting information (entry status, family situation, level of study, health point...) on the participant 2- phase of implementation of the action plan Mobilisation of existing stages/actions PLIE or not PLIE in relation to the objectives set: definition/validation and implementation of the project through guidelines on mobilisation actions. Mobilisations of financial aid PLIE or not PLIE as ISP, API...     Notre opération se déroule sur 3 territoires : Vénissieux, Vaulx-En- Velin,Rhône Sud et fait donc l'objet de 3 actions Le principe d'adhésion/d'engagement volontaire de la personne dans un parcours d'insertion socio professionnel renforcé et personnalisé est indispensable avant d'entreprendre toute démarche. A partir de l'adhésion du participant un travail est entrepris avec lui par le référent de parcours unique pour établir dans un premier temps un diagnostic approfondi social , personnel et professionnel pour évaluer sa capacité à adhérer à un accompagnement renforcé, à mettre en évidence ses motivations ,ses atouts et les problématiques repérées ainsi que les objectifs à fixer pour concrétiser son projet. Une fois son entrée PLIE validée en CSP nous allons établir avec le participant un plan d'action réaliste et progressif au vu de sa situation globale et conduire ce dernier a devenir acteur de son parcours. Le référent de parcours , personne ressource co construit et met en oeuvre dans le cadre de rencontres régulières avec le participant le parcours pertinent au regard des objectifs ciblés dans le diagnostic , et s'assure du bon déroulement de l'action. Le référent est le garant de l'accompagnement, de la mobilisation des actions/étapes et de l'évaluation régulière de celles-ci menées avec le référent d'étape.Le référent de parours va privilégié les diagnostics pluridisciplinaires et partagés. Il va rendre compte du parcours du participant dans les outils définis par le PLIE Pour mettre en place le plan d'action défini conjointement , le référent de parcours va permettre au participant d'être acteur de son parcours , notamment par un accès direct aux informations concernant les actions , les opportunités , les dispositifs d'aide proposés par le PLIE ( formation, mobilité, garde d'enfants ...) ou autres partenaires et par une participation à la mise en oeuvre du PLIE . Il va mobiliser ses réseaux, mettre en place des étapes, des actions pour solutionner les problématiques repérées du participant. Le référent va soutenir les démarches du participant par un accompagnement personnalisé évolutif en fonction de la réalité des besoins. Il va baliser, sécuriser au maximum le parcours du participant ( soutien pendant l'étape, prpération à la sortie de l'étape ) par un suivi régulier tant au niveau de l'étape que de l'emploi. Il participe à la dynamique du réseau ( emploi, formation, activité d'insertion par l'économie, mise en relation avec les CRE et la lutte contre les discriminations) . Il est à noter et c'est très important compte tenu de la typologie du public suivi que notre accompagnement n'est pas limité dans le temps ce qui permet d'ajuster le plan d'actions et les objectifs fixés en fonction de la progression du parcours d'insertion du participant. Le référent de parcours va assurer l'accompagnement du participant pendant les 6 premiers mois de sa prise de fonction sur un emploi de droit commun, création d'entreprise, validation d'une formation qualifiante. En résumé,l'accompagnement se décline de la façon suivante: 1- Phase accueil diagnostic Connaissance avec le participant, vérification de de son élégibilité, présentation du cadre et de la finalité du suivi et adhésion volontaire du participant Diagnostic approfondi partagé sur la base du CV et du vécu de la personne qui sera formalisé. Evaluation globale de la situation qui va nous permettre d'intentifier le projet professionnel. Formalisation des premières préconisations du plan d'action dans le contrat d'engagement joint au dossier de présentation PLIE et qui fera l'objet d'une présentation en CSP avec le questionnaire de recueil d'informations ( statut à l'entrée, situation familiale, niveau d'études, point santé ...) sur le participant 2- Phase de mis en oeuvre du plan d'actions Mobilisation des étapes/actions existantes PLIE ou non PLIE en lien avec les objectifs fixés : définition/validation et concrétisation du projet par des orientations sur des actions de mobilisation. Mobilisations des aides financières PLIE ou non PLIE comme FAI, API... Suivi dans les étapes ( contact téléphonique, bilan tripartite, entretien) pour évaluer la pertinence de l'action et la progression du participant et d'un réajustement si nécessaire.Le référent assure la mise en oeuvre et le suivi du parcours d'insertion en analysant la progression et en évaluant régulièrement avec le participant les objectifs propres à chacune des étapes du parcours.
