Proposer un accompagnement global pour une insertion par la santé aux personnes en situation de précarité sous deux axes: * Mettre en place un suivi santé individualisé des personnes en situation de précarité, par le biais d'entretiens, d'orientations et d'accompagnements physiques vers des praticiens ou lieux de soins adaptés aux pathologies repérés chez les usagers, en prenant en compte leur souffrance psychique. * Proposer à ce même public des ateliers collectifs de socialisation, permettant à la fois de travailler sur la valorisation de l'estime de soi et des compétences psychosociales, mais aussi de donner des informations de base sur la santé, les dispositifs d'accueil et l'accès aux soins. * Ces 2 axes de travail sont totalement complémentaires, chacun servant de passerelle à l'autre pour favoriser l'insertion par la santé.
