In view of the growing need for a growing group of patients at low, fixed amount of funding from national sources, i.e. the home of the Domestic Clearing House. The National Health Fund, the applicant assumes that three main assumptions are to be met as part of the project: “Comprehensive patient care in the framework of the Activity of Hospice in Zgierze” as part of its tasks: 1. Training for people in the vicinity of dependent persons, 2. Guardian of independent person, 3. Support to physiotherapist. Support for a self-employed person, in the framework of multifaceted training courses, support from a project employed as part of a project aimed at alleviating or replacing members of families of patients in the most vulnerable and social situations and increasing the services provided by a physiotherapist. All patients who will be registered in the Hospice House in Zgierz will be supported by the project, which will lead to a real improvement in the quality of the services provided to palliative hospice care patients. Only a holistic approach to the patient can bring positive outcomes for treatment and/or pain relief. The envisaged three-year duration of the project is to channel support to at least 525 people at risk and 525 people out of the surrounding area. (English)

Szkolenia dla osób z otoczenia osób niesamodzielnych, 2. Opiekun osoby niesamodzielnej, 3. Wsparcie fizjoterapeuty. Wsparcie otoczenia osoby niesamodzielnej w ramach wielopłaszczyznowych szkoleń, wsparcie ze strony Opiekuna zatrudnionego w ramach projektu, którego celem będzie odciążenie lub zastąpienie członków rodzin pacjentów w najtrudniejszej sytuacji ekonomiczno-społecznej oraz zwiększenie usług w ramach działalności fizjoterapeuty. Wszyscy pacjenci, którzy zostaną zarejestrowani w Hospicjum Domowym w Zgierzu otrzymają wsparcie w ramach projektu, które przełoży się na realne podniesienie jakości świadczonych usług dla pacjentów opieki paliatywno-hospicyjnej. Wyłącznie holistyczne podejście do chorego może przynieść pozytywne efekty leczenia i/lub uśmierzania bólu. Założony trzyletni okres trwania projekt zakłada skierowanie wsparcia dla co najmniej 525 osób zagrożonych oraz 525 osób z otoczenia osób niesamodzielnych. (Polish)
