PCM Sp z o.o. in Pabianice as an entity authorised to carry out a medical activity under the applicable law with a signed contract with the NFZ for primary health care, shall implement the project in accordance with the ROP ROP for 2014-2020 Priority IX ‘Social inclusion’, division IX.2.1 ‘Social and health services’. The actions will target pupils of classes of upper secondary education in the subregion, including those from families at risk of poverty and social exclusion. Participation is subject to the written consent of the parent/legal guardian on the child’s involvement in the project. Children will be excluded from the project: specialised medical care in respect of hearing disorders, financed by the National Health Fund (NFZ) and covered by a permanent speech therapy, financed by the National Health Fund. The population of parents/carers and teachers of classes I for the period 2018-2020 will also be covered by educational measures. Information and training activities will also be addressed to school nurses from the schools involved in the project. The project will cover the whole path of the planned interventions detailed in the PZ. The substantive actions will be preceded by information and publicity measures financed by the indirect costs. Each participant will be treated individually irrespective of their sex, religion, disability or belief. The hospital shall have architectural and information facilities adapted to the needs of persons with different disabilities and shall be carried out in accordance with the principles of equal opportunities and non-discrimination, the principle of sustainable development, the principle of universal design and the principle of equality between women and men. The project is in conformity with the type of project to be implemented as part of the competition for the rehabilitation of children at risk of disability and with disabilities — the implementation of health programmes. PCM Sp z o.o. w Pabianicach jako podmiot uprawniony do wykonywania działalności leczniczej na mocy obowiązującego prawa posiadający podpisaną umowę z NFZ w zakresie podstawowej opieki zdrowotnej, realizując projekt podejmie działania zgodne RPO WŁ na lata 2014-2020 Osią Priorytetową IX „Włączenie społeczne”, Poddziałaniem IX.2.1 „Usługi społeczne i zdrowotne”. Działania skierowane będą do uczniów I klas szkół podst. znajdujących się na terenie podregionu łódzkiego, w tym pochodzących z rodzin zagrożonych ubóstwem i wykluczeniem społecznym. Warunkiem uczestnictwa jest pisemna zgoda rodzica/opiekuna prawnego na udział dziecka w projekcie. Z udziału w projekcie wyłączone będą dzieci: objęte specjalistyczną opieką medyczną w zakresie zaburzeń słuchu, finansowaną ze środków NFZ oraz objęte stałą opieką logopedyczną, finansowaną ze środków NFZ. Działaniami edukacyjnymi będzie objęta również populacja rodziców/ opiekunów prawnych i wychowawców klas I w latach 2018 – 2020. Działania informacyjno - szkoleniowe będą także skierowane do pielęgniarek /higienistek szkolnych ze szkół biorących udział w Projekcie. Projekt będzie obejmować całą ścieżkę w ramach zaplanowanych interwencji szczegółowo opisanych w RPZ. Działania merytoryczne będą poprzedzone akcją informacyjno- promocyjną finansowaną w ramach kosztów pośrednich. Każdy uczestnik będzie traktowany indywidualnie bez względu na płeć, wyznanie, niepełnosprawność czy przekonania. Szpital dysponuje infrastrukturą architektoniczną i informacyjną dostosowaną dla potrzeb osób z różnymi niepełnosprawnościami a działania zawsze podejmowane są zgodnie z zasadą równości szans i niedyskryminacji, zasadą zrównoważonego rozwoju, zasadą projektowania uniwersalnego i zasadą równości kobiet i mężczyzn. Realizowany projekt jest zgodny z typem projektu przewidzianym do realizacji w ramach tego konkursu-rehabilitacji dzieci zagrożonych niepełnosprawnością i z niepełnosprawnościami – wdrożenie programów zdrowotnych.
