The “Autumn of the autumn for the Senora” project aims to increase the availability of high-quality care services in the local community for 15 dependent persons from Unilav by creating new ones in the DDP within the 01.09.2019-31.12.2020 deadline. The target group is 15 independent, disabled elderly people, socially excluded from the municipality of Unieslav. Persons who have a moderate or substantial degree of disability or secondary disabilities, intellectual disabilities, or persons with mental disorders, as well as persons who are not autonomous, will be eligible for the project. The following activities are planned: Adaptation of rooms, bathrooms and lifts for DDP, adaptation for people with disabilities, the provision of care services in the local community, rehabilitation services, activation and animation. 1. Number of facilities adapted to the needs of persons with disabilities [12]. The number of people at risk of poverty or social exclusion covered by the general interest social services in the programme: 153. The number of people at risk of poverty or social exclusion in the local community at programie:15B. Result indicators: 1. Number of social services existing at the end of the scheme supported by the scheme. The number of assistant and care services existing in the programme at the end of projektu:15Nad, the correct implementation of the project and the management team will be in charge of the project team (English) | |||

Celem projektu „Złota jesień dla Seniora – utworzenie dziennego domu pobytu dla osób starszych na terenie gminy Unisław" jest zwiększenie dostępności wysokiej jakości usług opiekuńczych świadczonych w społeczności lokalnej dla 15 osób niesamodzielnych z Gminy Unisław poprzez utworzenie nowych miejsc w DDP w terminie 01.09.2019 – 31.12.2020. Grupę docelową stanowią 15 niesamodzielnych, niepełnosprawnych osób starszych, wykluczonych społecznie z gminy Unisław. Do projektu zakwalifikowane zostaną osoby o umiarkowanym lub znacznym stopniu niepełnosprawności lub niepełnosprawnościami sprzężonymi, niepełnosprawnością intelektualną lub osób z zaburzeniami psychicznymi oraz osoby niesamodzielne. W projekcie zaplanowano następujące działania: Adaptację sal, łazienki oraz windy na potrzeby DDP, przystosowanie dla osób niepełnosprawnych, realizację usług opiekuńczych świadczonych w lokalnej społeczności, usług rehabilitacyjnych,realizacja aktywizacji i animacji społecznej.W projekcie realizowane będą wskaźniki:A. Wskaźniki produktu: 1. Liczba obiektów dostosowanych do potrzeb osób z niepełnosprawnościami [szt.] 12. Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie: 153. Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami asystenckimi i opiekuńczymi świadczonymi w społeczności lokalnej w programie:15B. Wskaźniki rezultatu: 1. Liczba wspartych w programie miejsc świadczenia usług społecznych istniejących po zakończeniu projektu:152.
