The project will run from 01.07.2020-30.06.2022 for a period of 24 months. Project objectives: Return to active function. 80 people in danger of poverty or social exclusion in the municipality of Torun, based on the application of the PAL activities and the launch of local activities and awareness about the forms of assistance available to 40 members of their entourage in 30.06.2022, aimed at 120 persons, including 80 people at risk of poverty or exclusion, (unemployed in the so-called ‘furthest’ group of work, people with disabilities, inactive people and others) and 40 people in distress. The persons covered will be provided in particular from degraded areas, i.e. from the foothills, Bydgoszcz of the suburbs and of the Old Town. In order to facilitate active inclusion in the field of social inclusion and reintegration into the labour market through a social contract and a local activity programme. In the framework of the initiative, action will be taken with the participation of activation, educational, social and health activation instruments. The process of support for people, families and environments at risk of poverty or social exclusion will be based on a reintegration path established for each person, family, environment at risk of poverty or social exclusion, taking into account the diagnosis of problem, resources, capabilities, predisposition and needs. Support is focused on people and their needs rather than on developing services. Take into account the situation on the local labour market. The MOPSC of the project shall cooperate closely with the PLO for MT. The requirement is 34 % social efficiency and employment efficiency of 25 % and 12 % (persons with disabilities). Projekt będzie realizowany w okresie od 01.07.2020-30.06.2022 r. przez okres 24 m-cy. Celem projektu jest: Powrót do aktywnego funkcj. społ. i zaw. 80 osób zagroż. ubóstwem lub wykluczeniem społ., zamieszk. na terenie Gminy M.Toruń w oparciu o zastosowanie działań PAL oraz inicjowania aktywności lokalnej i świadomości nt. dostępnych form pomocy u 40 członków otoczenia tych osób do 30.06.2022 r. Proj. skierowany jest do 120 osób, w tym 80 osób zagrożonych ubóstwem lub wyklucz. społ., (osoby bezrobotne w tzw. grupie najbardziej oddalonej od r.pracy, osoby niepełnosprawne, osoby bierne zawodowo i in.) oraz 40 osób z otoczenia osób zagroż. wykluczeniem społ. Osoby objęte wsparciem pochodzić będą w szczególności z obszarów zdegradowanych, objętych rewitalizacją tj. z Podgórza, Bydgoskiego Przedmieścia oraz Starego Miasta. W proj. prowadzone będą zindywidualizowanie i kompleksowe działania umożliwiające aktywne włączenie społeczne, a także powrót na rynek pracy realizowane poprzez kontrakt socjalny oraz Program Aktywności Lokalnej. W ramach proj. będą podejmowane działania z udziałem instrumentów aktywizacji zawodowej, edukacyjnej, społecznej oraz o charakterze zdrowotnym. Proces wsparcia osób, rodzin i środowisk zagrożonych ubóstwem lub wykluczeniem społecznym odbywać się będzie w oparciu o ścieżkę reintegracji, stworzoną indywidualnie dla każdej osoby, rodziny, środowiska zagrożonych ubóstwem lub wykluczeniem społecznym, z uwzględnieniem diagnozy sytuacji problemowej, zasobów, potencjału, predyspozycji i potrzeb. Wsparcie w ramach proj. koncentrowane jest na osobie i jej potrzebach, nie zaś na rozwijaniu usług. Proj. uwzględnia sytuację na lokalnym rynku pracy. MOPR będący realizatorem projektu ściśle współpracuje z PUP dla MT. Proj. zakłada efektywność społ. na poziomie 34% oraz efektywność zatrudnieniową na poziomie 25% i 12% (osoby z niepełnosprawnościami). Realizowany proj. jest zgodny z Krajowym Programem Przeciwdziałania Ubóstwu i Wykluczeniu Społecznemu 2020. (Polish)
