“Domowa marina” — a project aimed at developing a new model of support based on the transfer of solutions applied by the foreign partner in the care, care, prevention of the elderly.The new support model is a product which, in part and in a given form, has so far not existed in Poland.The support model concerns the extension and improvement of the quality of care, care and prevention services that are characteristic of DDP clients.The project product will be a model that can be used by DDP operators in Poland, with detailed instructions for implementation.The model will focus on:nursing care (educational previsions, working methods for the care of the elderly or affected by conditions, training for the care of the elderly or for the families of DDP clients, the training of DDP employees and the families of DDP clients, the provision of emotional support to the customer and its families, the provision of emotional support to the customer and his family, the provision of emotional support to the customer and his family, training in care methods for DDP clients, training courses for the families of DDP clients, the training of DDP’s family families, training of clients of DDP clients as regards care methods for the elderly and the way in which they communicate with them), the prevention of medical conditions (popularising the health behaviour of clients and their families, training by geriatric doctors, psychiatrists). the model developed will be guided by instructions, training scenarios for all the above activities.Transnational cooperation provides for the transfer, adoption and implementation of a new solution at national level, which is taken from a supranational partner.Furthermore, an integral part of this cooperation at every stage of the project is the exchange of information and experience with a supranational partner and will be financed on the basis of reciprocity. "Domowa przystań" - projekt mający na celu opracowanie nowego modelu wsparcia w oparciu o transfer rozwiązań stosowanych przez partnera zagranicznego w zakresie pielęgnacji, opieki, profilaktyki schorzeń osób starszych. Nowy model wsparcia to produkt, który w części i w określonej formie nie występuje dotychczas w Polsce. Model wsparcia dotyczy rozszerzenia i poprawy jakości oferty usług w zakresie pielęgnacji, opieki i profilaktyki schorzeń charakterystycznych dla klientów DDP. Produktem projektu będzie model możliwy do wykorzystania przez podmioty prowadzące DDP na terenie Polski, zawierający szczegółowe instrukcje wdrożenia. Model będzie dotyczył zakresów: pielęgnacji (prelekcje edukacyjne, metody pielęgnacji osób starszych lub obarczonych schorzeniami, szkolenia z metod pielęgnacji dla pracowników DDP i dla rodzin klientów DDP, szkolenia pracowników DDP z edukowania rodzin klientów DDP w zakresie metod pielęgnacji osób starszych i sposobu komunikowania się z osobami starszymi), opieki (prelekcje edukacyjne, metody pracy z osobami starszymi lub obarczonymi schorzeniami, udzielanie wsparcia emocjonalnego klientowi i jego rodzinie, szkolenia z metod opieki dla pracowników DDP, szkolenia z metod opieki dla rodzin klientów DDP, szkolenia pracowników DDP z edukowania rodzin klientów DDP w zakresie metod opieki nad osobami starszymi i sposobu komunikowania się z nimi), profilaktyki schorzeń (popularyzowanie zachowań prozdrowotnych wśród klientów i ich rodzin, szkolenia prowadzone przez lekarzy geriatrów, psychiatrów).W opracowanym modelu znajdą się instrukcje, scenariusze szkoleń do wszystkich powyższych działań. Współpraca ponadnarodowa przewiduje transfer, zaadoptowanie i wdrożenie nowego rozwiązania na gruncie krajowym, zaczerpnięte od partnera ponadnarodowego. Ponadto nieodłącznym elementem tej współpracy na każdym etapie projektu jest wymiana informacji i doświadczeń z partnerem ponadnarodowym i będzie finansowana na zasadzie wzajemności. (Polish)
