Los primeros datos facilitados por el registro poblacional de cáncer de la provincia de Huelva sobre cáncer de mama muestran unos valores de incidencia en línea con los del conjunto de España. Sin embargo, esta provincia viene mostrando una mortalidad por este tipo de cáncer más elevada que la del conjunto de la nación. Un acceso ineficiente por parte de la población al sistema de salud, o a factores estructurales en la organización de la atención socio-sanitaria a los pacientes sospechosos de padecer cáncer de mama se asocian a una mayor letalidad. Los objetivos del presente estudio son: (1) Identificar las posibles causas relacionadas al proceso asistencial de una mayor letalidad por cáncer de mama en una provincia con elevada mortalidad por cáncer (Huelva). (2) Estudiar la asociación entre variables clínico-asistenciales y la duración de los intervalos “fecha del diagnóstico – fecha del inicio del tratamiento”, y “fecha de cirugía – fecha de inicio de tratamiento adyuvante” en cáncer de mama en Huelva. (3) Estudiar la asociación entre nivel socio-cultural y los intervalos “fecha del diagnóstico – fecha del inicio del tratamiento”, “fecha de cirugía – fecha de inicio de tratamiento adyuvante”, y “primer síntoma-inicio del tratamiento” en cáncer de mama en el estudio MCC-Spain. Se combinará metodología cualitativa y cuantitativa con entrevistas en profundidad, grupos focales entre profesionales de la salud relacionados con el proceso asistencial en cáncer en Huelva, y con pacientes de cáncer de mama. En el periodo 2007-2011, el registro de cáncer de Huelva ha identificado 1312 casos de cáncer de mama. Para la asociación entre variables socioculturales y los intervalos de interés se utilizarán datos del estudio MCC-Spain (1750 casos de cáncer de mama), de los que 115 son del nodo de Huelva.Para el segundo objetivo se utilizarán los datos del registro de cáncer de Granada (como referencia de provincia con razón incidencia/mortalidad favorable en cáncer de mama (Spanish)

Inefficient access by the population to the health system, or structural factors in the organisation of socio-health care for patients suspected of breast cancer are associated with increased lethality. The objectives of this study are to: (1) Identify possible causes related to the care process of increased breast cancer lethality in a province with high cancer mortality (Huelva). (2) Study the association between clinical-care variables and the duration of the intervals “date of diagnosis — date of start of treatment”, and “date of surgery — date of initiation of adjuvant treatment” in breast cancer in Huelva. (3) Study the association between socio-cultural level and the intervals “date of diagnosis — date of start of treatment”, “date of surgery — date of initiation of adjuvant treatment”, and “first symptom-start of treatment” in breast cancer in the MCC-Spain study. Qualitative and quantitative methodology will be combined with in-depth interviews, focus groups among health professionals related to the cancer care process in Huelva, and with breast cancer patients. In the period 2007-2011, the Huelva cancer registry has identified 1312 cases of breast cancer. Data from the MCC-Spain study (1750 cases of breast cancer), of which 115 are from the Huelva node, will be used for the association between sociocultural variables and interest intervals.For the second objective, data from the Granada cancer registry will be used (as a provincial reference with incidence/mortality ratio favorable in breast cancer).
