Subcutaneous adipose tissue is capable of expanding in situations of positive energy balance. However, the expansion capacity is limited and once exceeded there is an ectopic deposition of lipids in other organs that is the origin of metabolic disorders associated with obesity. OBJECTIVES: 1. Study of the biogenesis of subcutaneous adipose tissue, to determine the factors that fix its expansion limit. 2. Establish non-invasive biomarkers that can be used in clinical practice to differentiate obese individuals in whom the limit of expansion has not yet been reached (benign obesity) from those who have already exceeded the limit of expansion and who will require aggressive therapy. METHODOLOGY: Patients from the Surgery Service who donate a sample of subcutaneous fat (prospective study) will be included. Abdominal imaging techniques will be performed to determine the level of expansion of its adipose tissue, metabolomic blood and fat to determine the metabolites produced by the adipocyte in the expansion process and transcriptomics in the adipose tissue to determine changes in the expression of genes related to the adipogenic process. Biomarkers of interest will be tested in vitro and in a validation cohort. These biomarkers will reduce the economic and care burden of obesity on society as a whole by allowing the early detection of pathological obesity. (English)

    La obesidad es una enfermedad crónica de origen multifactorial que se define como una acumulación de grasa que causa problemas de salud. El tejido adiposo subcutáneo es capaz de expandirse en situaciones de balance energético positivo. Sin embargo, la capacidad de expansión es limitada y una vez superada se produce una deposición ectópica de lípidos en otros órganos que es el origen de los trastornos metabólicos asociados a la obesidad. OBJETIVOS: 1. Estudio de la biogénesis del tejido adiposo subcutáneo, para determinar los factores que fijan su límite de expansión. 2. Establecer biomarcadores no invasivos que se puedan utilizar en la práctica clínica para diferenciar los individuos obesos en los que el límite de expansión aún no se haya alcanzado (obesidad benigna) de los que ya han sobrepasado el límite de expansión y que requerirán una terapia agresiva. METODOLOGIA: Se incluirán pacientes procedentes del Servicio de Cirugía que donaran una muestra de grasa subcutánea (estudio prospectivo). Se realizaran técnicas imagen abdominal para determinar el nivel de expansión de su tejido adiposo, metabolómica en sangre y grasa para determinar los metabolitos producidos por el adipocito en el proceso de expansión y transcriptómica en el tejido adiposo para determinar cambios en la expresión de genes relacionados con el proceso adipogénico. Los biomarcadores de interés serán testados in vitro y en una cohorte de validación. Estos biomarcadores disminuirán la carga económica y asistencial de la obesidad sobre el conjunto de la sociedad al permitir la detección precoz de la obesidad patológica. (Spanish)
