The fragmentation of health services has a detrimental effect, especially for chronic patients, since, in addition to requiring assistance from professionals from various specialties, they establish a long-term relationship with the health system. Patients in the locomotive area have resolvable chronic pathologies that require long-term attention, so they suffer from problems of access, continuity, fragmentation and quality. Traditionally, care integration amounts to coordination between care levels that is theoretically guaranteed through the use of clinical practice guidelines. However, on many occasions the clinical information of the patient is not available to any professional and the clinical criteria are different at each level. In addition, limited access to diagnostic tests slows the resolution of problems at the primary level of care. The creation of Integrated Health Organisations (OSIs) was a patient-oriented management approach, with routes being the main support tool for this purpose. The care routes of the osteo-muscular area integrate primary care, traumatology, rehabilitation and rheumatology. This project aims to evaluate an integrated model of management of the pathology of the locomotive system in an integrated health organisation that includes the osteo-muscular route. Simulation with discrete events will be used to obtain performance indicators in each scenario. (English)

    La fragmentación de los servicios sanitarios tiene un efecto perjudicial, especialmente el los pacientes crónicos, puesto que, además de requerir asistencia de profesionales de diversas especialidades, establecen una relación a largo plazo con el sistema sanitario. Los pacientes del área locomotor presentan patologías crónicas resolubles que requieren atención durante largo tiempo por lo que sufren los problemas de acceso, continuidad, fragmentación y calidad. Tradicionalmente, la integración asistencial equivale a la coordinación entre los niveles asistenciales que teóricamente se garantiza mediante el uso de guías de práctica clínica. Sin embargo, en muchas ocasiones la información clínica del paciente no está disponible para cualquier profesional y los criterios clínicos son diferentes en cada nivel. Asimismo, la limitación al acceso de pruebas diagnósticas frena la resolución de problemas en el nivel primario de atención. La creación de Organizaciones Sanitarias Integradas (OSIs) supuso un enfoque de gestión orientada al paciente, siendo las rutas la principal herramienta de apoyo para este fin. Las rutas asistenciales del área osteo-muscular integran atención primaria, traumatología, rehabilitación y reumatología. Este proyecto se dirige a evaluar un modelo integrado de gestión de la patología del aparato locomotor en una organización sanitaria integrada que incluye la ruta osteo-muscular. Se utilizará la simulación con eventos discretos para obtener los indicadores de rendimiento en cada escenario. (Spanish)
