The objective of this project is to develop an innovative strategy for the management of chronic patients, consisting of an integrated and proactive care intervention supported by a strong component of the use of information technologies, combining the use of predictive models for the identification of candidates, remote and intelligent patient management, and the tools to support decision-making (a product we will call ehealth technology) as well as the validation of its use in the usual practice to ensure its general applicability and transfer to the healthcare sector. This general objective is structured into two sub-projects: (COORDINATED PROJECT 1; DTS14/00123) and (COORDINED PROJECT 2; DTS14/00185). In recent years the Conselleria de Sanitat of the Valencian Community has launched two relevant experiences in the management of chronic patients. These experiences have been the GeCHRONIC program in the Department of Health Valencia-La Fe and the ValCR▷NIC program in the Health Departments of Sagunt and Elx. The (even partial) evaluations of both programs indicate that they are relevant, contrasted and viable initiatives that achieve significant improvements in health outcomes, and in the satisfaction of patients and caregivers and, consequently, with a high transfer potential at both national and international levels. This project aims to introduce innovations in the development of “ehealth technology” used in previous experiences, based on the experience and identification of improvements detected to ensure usability and transferability of technology to different contexts.     El objetivo de este proyecto es el desarrollo de una estrategia innovadora para la gestión de los pacientes crónicos, consistente en una intervención asistencial integrada y proactica apoyada en un fuerte componente de uso de las tecnologías de información, combinando el uso de modelos predictivos para la identificación de candidatos, la gestión remota e inteligente de los pacientes, y las herramientas de apoyo a la toma de decisiones (producto que en adelante denominaremos tecnología ehealth ) así como la validación de su uso en la práctica habitual para asegurar su aplicabilidad general y transferencia al sector sanitario. Este objetivo general se estructura en dos subproyectos: (PROYECTO COORDINADO 1; DTS14/00123) y (PROYECTO COORDINADO 2; DTS14/00185). En los últimos años la Conselleria de Sanitat de la Comunidad Valenciana ha puesto en marcha dos experiencias relevantes en la gestión de pacientes crónicos. Están experiencias han sido el programa GeCHRONIC en el Departamento de Salud Valencia-La Fe y el programa ValCRÒNIC en los Departamentos de Salud de Sagunt y de Elx. Las evaluaciones (aun parciales) de ambos programas apuntan que son iniciativas relevantes, contrastadas y viables que consiguen importantes mejoras en resultados de salud, y en la satisfacción de pacientes y cuidadores y, en consecuencia, con un alto potencial de transferencia tanto a nivel tanto nacional como internacional. Este proyecto pretende introducir innovaciones en el desarrollo de la “tecnología ehealth” utilizada en experiencias previas, en base a la experiencia e identificación de mejoras detectadas para asegurar usabilidad y transferibilidad de la tecnología a diferentes contextos.
