The metabolic effects of lipid emulsions of parenteral nutrition (NP) depend on the composition of fatty acids, which have different action on plasma lipids, inflammatory status and oxidation, thus acting in the clinical course by modulating the immune response. This study compares two different types of lipids (rich in oleic acid vs lipid enriched in fish oil (ClinOleic v SMOF-LIPID) to check the different action on plasma lipids, and in oxidation, clinical course, complications, hospital stay. The objective is to demonstrate non-equality in lipid fractions, and secondaryly difference in biochemical parameters and clinical evolution. Methodology A prospective, randomised, blind study in 80 surgical patients admitted to ICU, with PN: 40 will receive eBao and 40 SMOF lipid. The following shall be analysed: lipid fractions, antioxidants, liver enzymes, nutritional parameters, every 7 days until the completion of parenteral nutrition or up to 28 days, following the study in the Surgery plant. Clinical data on infectious complications (catheter-related infection, surgical wound infection, intraabodominal absceum or peritonitis, pneumonia associated with mechanical ventilation) b.- organ dysfunction and expected mortality shall be collected; c.- side effects, d.- hospital stay. Follow-up to hospital discharge and 6 months post discharge. Multidisciplinary study: Queen Sofia General Hospital. Intensivist: initiates the NP; Pharmacy: randomisation and preparation NP; Nutrition Unit: monitoring in the NP plant; Department of Food Technology (UCAM): Inflammatory status marker analysis.     Los efectos metabolicos de las emulsiones lipidicas de la nutrición parenteral (NP) dependen de la composición de acidos grasos, que tienen acción distinta sobre los lípidos plasmáticos, el estado inflamatorio y oxidación, actuando así en el curso clínico al modular la respuesta inmune. En este estudio se comparan dos tipos distintos de lipidos (ricos en acido oleico vs lipido enriquecido en aceite de pescado (ClinOleic v SMOF-LIPID) para comprobar la distinta acción sobre los lípidos plasmáticos, y en estado de oxidación, curso clínico, complicaciones, estancia hospitalaria. El objetivo es demostrar la no igualdad en fracciones lipídicas, y de forma secundaria diferencia en parámetros bioquímicos y de evolución clínica . Metodologia Estudio prospectivo, randomizado , ciego en 80 pacientes quirúrgicos ingresados en UCI, con NP: 40 recibirán EBAO y 40 SMOF lipid. Se analizarán: fracciones lipídicas, antioxidantes, enzimas hepáticas, parámetros nutricionales, cada 7 días hasta la terminación de la nutrición parenteral o hasta 28 días, siguiendo el estudio en planta de Cirugía. Se recogerán los datos clínicos de a.- complicaciones infecciosas ( infección relacionada con catéter, infección de herida quirúrgica, absceo intraabodominal y/o peritonitis, neumonía asociada a ventilación mecánica) b.- disfunción de órganos y mortalidad esperada; c.- efectos adversos, d.- estancia hospitalaria. Seguimiento al alta hospitalaria y 6 meses post alta. Estudio multidisciplinar: Hospital General Reina Sofía. Intensivista: inicia la NP; Farmacia: aleatorización y preparación NP; Unidad de Nutrición: seguimiento en planta de NP; Departamento de Tecnología de alimentos (UCAM): Análisis marcadores de estado inflamatorio.
