The SmartPites project seeks the design, development and deployment of a platform for the management of integrated mental health care services, specifically the management of the provision of Personal Recovery Plans for people with severe mental disorder. The aim is to place the user at the centre of the rehabilitation process, where a community mental health and recovery approach prevails. The resulting platform will allow the collection of social and health information, bringing together and allowing the creation of a social history, and integrating it into the patient’s medical history. The platform should allow the interoperability of information to ensure continuity of care between the actors involved, not only within the health system (primary, mental health) but also in social institutions providing social care. It will also encourage collaboration between the actors involved through communication and information technologies. Reasons for collaboration between health and social systems, improvement of comprehensive patient care, sustainability of welfare systems and the search for excellence in the quality of the services offered have inspired the Aragones de Salud Service in the design of this project. Involving social agents in the provision of integrated patient-oriented care with Serious Mental Disorder. El proyecto SmartPites busca el diseño, desarrollo y despliegue de una plataforma para la gestión de los servicios de cuidados integrados en Salud Mental, en concreto, la gestión de la provisión de Planes Personales de Recuperación en personas con trastorno mental grave. Se busca situar al usuario en el centro del proceso rehabilitador, donde prevalezca un enfoque de salud mental social comunitaria y de recuperación. La plataforma resultante permitirá recopilar información social y sanitaria, aunando y permitiendo la creación de una historia social, e integrándola en la historia clínica del paciente. La plataforma deberá permitir la interoperabilidad de la información para asegurar una continuidad asistencial entre los agentes involucrados, no solo dentro del sistema sanitario (primaria, salud mental) sino también en las entidades sociales proveedores de atención social. Así mismo, fomentará la colaboración entre los actores implicados a través de las tecnologías de comunicación e información. Razones de colaboración entre los Sistemas Sanitarios y Sociales, mejora de la atención integral de los pacientes, de sostenibilidad de los Sistemas del Bienestar y la búsqueda de la excelencia en la calidad de los servicios ofertados han inspirado al Servicio Aragones de Salud en el diseño de este proyecto. Involucrando a los agentes sociales en la prestación de cuidados integrados orientados al paciente con Trastorno Mental Grave. Este proyecto SmartPITES exp PI18/01157 forma parte del proyecto coordinado en red de 6 nodos (PI18CIII/00004, PI18/00700, PI18/00841, PI18/00788, PI18/01047, PI18/01157) enmarcado en el subprograma de proyectos de investigación en evaluación de tecnologías sanitarias y servicios de salud.
