The provision of care services in the social services system at community level is an effective support tool to ensure that clients dependent on the assistance of others remain in their natural environment, which is a key prerequisite for reducing poverty and social exclusion. The aim of this project is to support the development of care services in order to prevent clients from being placed in residential social services. Implementation of the project will take place through the main activity of the project by providing a care service within the meaning of the law on social services by qualified caregivers. Through the implemented main activity umožníme:podporu the development of care services in order to prevent the placement of clients in social services and support community care,provision of care services to more people dependent on help of another person in the home environment, staying the client of social services, for as long as possible in the natural environment in the family and community, increase the availability of care services for disadvantaged groups of the population (children and natural persons with severe disabilities, with adverse health status, seniors, people with social exclusion), create a suitable environment for the preservation, restoration and development of the capacity of natural persons and their families to lead an independent life, create and maintain care workers’ jobs and thereby promote employment in Trstená, support information campaigns to provide the public with information on the provision of care services and deinstitutionalisation. The whole project will be carried out for 26 months in the city of Trstená, in the natural home environment of the client, where he will be provided a care service by a qualified caregiver providing services in the field of social inclusion.Measurable indicators projektu:Počet supported capacities of new innovative services or measures at community level; in a home environment, open environment or substitute environment – 7.Number of projects targeting public administrations or social services at national, regional and local level – 1. Poskytovanie opatrovateľskej služby v systéme sociálnych služieb na komunitnej úrovni je efektívnym nástrojom podpory na zabezpečenie zotrvania klientov odkázaných na pomoc iných v ich prirodzenom prostredí, čo je hlavným predpokladom pre znižovanie chudoby a miery sociálneho vylúčenia.Cieľom predkladaného projektu je podpora rozvoja opatrovateľskej služby za účelom predchádzania umiestňovania klientov do pobytových zariadení sociálnych služieb. Realizácia projektu bude prebiehať prostredníctvom hlavnej aktivity projektu a to poskytovaním opatrovateľskej služby v zmysle zákona o sociálnych službách kvalifikovanou opatrovateľkou.Prostredníctvom realizovanej hlavnej aktivity umožníme:podporu rozvoja opatrovateľskej služby za účelom predchádzania umiestňovania klientov do zariadení sociálnych služieb a podpora komunitnej starostlivosti,poskytovanie opatrovateľskej služby väčšiemu počtu osôb odkázaných na pomoc inej osoby v domácom prostredí,zotrvanie klienta sociálnych služieb, čo najdlhšiu dobu v prirodzenom prostredí v rodine a komunite,zvýšiť dostupnosť opatrovateľskej služby pre znevýhodnené skupiny obyvateľstva (deti a fyzické osoby s ťažkým zdravotným postihnutím, s nepriaznivým zdravotným stavom, seniorom, osoby postihnuté sociálnym vylúčením),vytvoriť vhodné prostredie pre zachovanie, obnovu a rozvoj schopnosti fyzických osôb a ich rodín viesť samostatný život,vytvoriť a udržať pracovné miesta opatrovateliek a tým podporiť zamestnanosť v meste Trstená,podpora informačných kampaní s cieľom poskytnúť verejnosti informácie o poskytovaní opatrovateľskej službe a deinštitucionalizácii.Celý projekt bude realizovaný 26 mesiacov v mesta Trstená, v prirodzenom domácom prostredí klienta, kde sa mu bude poskytovať opatrovateľská služba kvalifikovanou opatrovateľkou poskytujúca služby v oblasti sociálneho začlenenia.Merateľnými ukazovateľmi projektu:Počet podporených kapacít nových inovatívnych služieb alebo opatrení na komunitnej úrovni, v domacom prostredí, otvorenom prostredí alebo náhradnom prostredí – 7.Počet projektov zameraných na verejné správy alebo sociálne služby na vnútroštátnej, regionálnej a miestnej úrovni – 1.
