The aim of the project “Home Care Service in Trenčín Region” is the deinstitutionalisation of social care and the transition from institutional to community-based care. The emphasis is on making quality social care available to the public in accordance with Act No 448/2008 on social services with regard to sustainability and accessibility. Through the main activity of providing care services, the project aims to develop social services and measures to prevent clients from being placed in social services. The target group is natural persons dependent on the assistance of another natural person to whom social services are provided and employees implementing social inclusion policies and measures in both the public and non-public sectors. The place of implementation of the project is Trenčiansky kraj. In Trencin region we plan to create two jobs for the main employment (full time). The monitored measurable indicators are the numbers of supported capacities of new innovative services or measures at community level, in a home environment open or in a substitute environment (P0334) (measurable indicator is 2.00) and the number of projects targeting public administrations or social services at national, regional and local level (P0361) (measurable indicator is 1.00)." The purpose of the project is to improve care for people dependent on assistance in the home environment, to improve the quality of life of both the family and the community from the point of view of security and safety in the provision of community-based care that significantly contributes to raising living standards. Cieľom projektu "Domáca opatrovateľská služba v Trenčianskom kraji" je deinštitucionalizácia sociálnej starostlivosti a prechod z inštitucionálnej na komunitnú starostlivosť. Dôraz je kladený na sprístupnenie kvalitnej sociálnej starostlivosti v zmysle zákona č. 448/2008 o sociálnych službách verejnosti s ohľadom na udržateľnosť a dostupnosť. Prostredníctvom hlavnej aktivity "poskytovania opatrovateľskej služby" projekt sleduje zámer rozvoja sociálnych služieb a opatrení za účelom predchádzania umiestňovania klientov do zariadení sociálnych služieb. Cieľovou skupinou sú fyzické osoby odkázané na pomoc inej fyzickej osoby, ktorým sa poskytujú sociálne služby a zároveň zamestnanci vykonávajúci politiky a opatrenia v oblasti sociálneho začlenenia vo verejnom aj neverejnom sektore. Miesto realizácie projektu je Trenčiansky kraj. V Trenčianskom kraji plánujeme vytvoriť dve pracovné miesta, na hlavný pracovný pomer (celý úväzok). Sledovanými merateľnými ukazovateľmi sú počty podporovaných kapacít nových inovatívnych služieb alebo opatrení na komunitnej úrovni, v domácom prostredí otvorenom prostredí alebo náhradnom prostredí (P0334) (merateľný ukazovateľ je 2,00) a počet projektov zameraných na verejné správy alebo sociálne služby na vnútroštátnej, regionálnej a miestnej úrovni (P0361) (merateľný ukazovateľ je 1,00).“ Zmyslom projektu je skvalitnenie starostlivosti o osoby odkázané na pomoc v domácom prostredí, zvýšenie kvality života rodiny i komunity z pohľadu istoty a bezpečnosti pri poskytovaní komunitnej starostlivosti ktorá výrazne prispieva k zvyšovaniu životnej úrovne.
