Το έργο αφορά στην συνέχιση της λειτουργίας και αναβάθμιση των υπηρεσιών που παρέχονται από το Κέντρο Διημέρευσης Ημερήσιας Φροντίδας για ’τομα με ειδικές ανάγκες, Νοητική Υστέρηση του Συλλόγου «ΑΓΑΠΗ», στο Ρέθυμνο, για 3 έτη και αφορά κατ΄ ελάχιστο τις παρακάτω ενέργειες:α) Την παροχή υπηρεσιών ημερήσιας φροντίδας και παραμονής σε άτομα με αναπηρίες, οι οποίες περιλαμβάνουν:Τη μεταφορά των ωφελουμένων προς και από το Κέντρο, με μεταφορικό μέσο του δικαιούχου.Τη διαμονή και τη διατροφή τους (πρόχειρο γεύμα). Το πρόγραμμα πράξεων ειδικής αγωγής που έχουν ανάγκη ανάλογα με τις ανάγκες του κάθε ΑμεΑ (εργοθεραπείες, φυσικοθεραπείες και άλλα), βάσει του εξατομικευμένου προγράμματος του κάθε ωφελούμενου.Την παροχή ατομικής ή και ομαδικής άσκησης.Την εκπαίδευση τους στην αυτοεξυπηρέτηση και την εκμάθηση δραστηριοτήτων καθημερινής ζωής. Τη δημιουργική απασχόλησή τους και δραστηριότητες κοινωνικοποίησής τους.Τη συμμετοχή τους σε προγράμματα ψυχαγωγίας, πολιτισμού και άθλησης.Την υλοποίηση δράσεων δικτύωσης και συνεργασίας με κοινωνικούς φορείς / φορείς παροχής κοινωνικών υπηρεσιών, τα υπό ίδρυση Κέντρα Κοινότητας, άλλες δομές παροχής παρεμφερών υπηρεσιών και την τοπική κοινότητα γενικότερα, με στόχο τη διασύνδεση των δομών με την τοπική κοινότητα, την ένταξη των ωφελουμένων σε αυτήν και συνεπώς την ενίσχυση της κοινωνικής συνοχής σε τοπικό επίπεδο. Οι ωφελούμενοι είναι άτομα ενήλικες και των δυο φύλων με Νοητική Υστέρηση (ασκήσιμα και εκπαιδεύσιμα) και ελαφρές συνοδές αναπηρίες (όρασης ακοής κινητικές κλπ.), καθώς και αυτιστικά άτομα οι οποίοι θα λαμβάνουν τις προαναφερόμενες παρεχόμενες υπηρεσίες στα ανωτέρω κέντρα. Το Κέντρο μπορεί να δεχτεί μέχρι 35 άτομα, 18 50 ετών, σύμφωνα με την άδεια λειτουργίας του, όμως στη συγκεκριμένη πράξη πρόκειται να ενταχθούν 25 άτομα. (Greek)

The project concerns the continuation of the operation and upgrading of the services provided by the Day Care Center for People with Disabilities, Mental Deprivation of the “AGAPI” Association, in Rethymno, for 3 years and relates to at least the following actions: a) The provision of daily care and accommodation services to people with disabilities, which include:The transfer of the beneficiaries to and from the Centre, by means of transport of the beneficiary and the beneficiary. The program of special education actions that are in need according to the needs of each disabled person (ergotherapy, physiotherapies and others), based on the individualised program of each beneficiary.The provision of individual and/or group exercise.The training in self-service and learning activities of everyday life. Their creative occupation and socialisation activities.Their participation in entertainment, culture and sports programmes.The implementation of networking and cooperation with social bodies/social service providers, the Community Centres, other similar services structures and the local community in general, with the aim of connecting the structures with the local community, integrating the beneficiaries into it and thus strengthening social cohesion at local level. The beneficiaries are adults of both sexes with Cognitive Disability (motorable and educable) and light accompanying disabilities (hearing vision motors etc.), as well as autistic individuals who will receive the aforementioned services in the above mentioned centres. The Centre can accept up to 35 people, 18 years of age, according to its authorisation, but 25 people are to be included in this act. (English)
