Support the process of deinstitutionalisation and prevention of re-institution of adults with disabilities within the Center for Rehabilitation and Rehabilitation of Persons with Disabilities Comanesti by providing services at community level. This objective will be achieved by providing services within protected dwellings and the day care center established by the project “Community services for adults with disabilities” financed by ROP 2014-2020, MySMIS 2014+: 119327, and through social services created as a result of the establishment of the network of professional personal assistants and the Support and Support Service for adults with deinstitutionalised disabilities within the Center for Recovery and Rehabilitation of Persons with Disabilities Comanesti. The social services will be taken over in the organisation chart, the state of office and the budget of DGASPC Bacau. During the project implementation period, the effective functioning of the social services will be ensured by the recruitment, selection and employment of staff, elaboration of working procedures with beneficiaries, implementation of intervention plans, licensing of services. Through the proposed activities, the project contributes to achieving OS 4.15, Axis 4 of POCU "Reduction of the number of elderly people and those with disabilities placed in residential institutions, by providing social and medical services at community level, including long-term services. Protected dwellings will provide family-type hosting in a protected system, personal care, assistance and support to beneficiaries, and within the day care center will be organised activities: social counseling, psychological counseling, empowerment and rehabilitation activities, etc. Beneficiaries placed with professional personal assistant will benefit from housing services, personal care, feeding with support offered through the Support and Support Service for adults with disabilities: social information and counselling, psychological counselling, individual support/support groups, assistance and support for making a decision. All these activities are carried out in order to prevent the reinstitutionalisation of adults with deinstitutionalised disabilities by acquiring new skills, increasing self-esteem and self-confidence. Sustinerea procesului de dezinstitutionalizare si de prevenire a reinstitutionlizarii persoanelor adulte cu dizabilitati din cadrul Centrului de Recuperare si Reabilitare a Persoanelor cu Handicap Comanesti prin asigurarea serviciilor la nivelul comunitatii. Acest obiectiv va fi atins prin oferirea de servicii in cadrul locuintelor protejate si a centrului de zi infiintate prin proiectul „Servicii comunitare pentru persoane adulte cu dizabilitati” finantat prin POR 2014-2020, MySMIS 2014+: 119327, si prin serviciile sociale create ca urmare a infiintarii retelei de asistenti personali profesionisti si a Serviciului Asistenta si Suport pentru persoanele adulte cu dizabilitati dezinstitutionalizate din cadrul Centrului de Recuperare si Reabilitare a Persoanelor cu Handicap Comanesti. Serviciile sociale vor fi preluate in organigrama, statul de functii si bugetul DGASPC Bacau. In perioada de implementare a proiectului se va asigura functionarea efectiva a serviciilor sociale realizand recrutarea, selectia si angajarea personalului, elaborarea procedurilor de lucru cu beneficiarii, implementarea planurilor de interventie, licentierea serviciilor. Prin activitatile propuse, proiectul contribuie la atingerea OS 4.15, Axa 4 din cadrul POCU "Reducerea numarului persoanelor varstnice si a celor cu dizabilitati plasate in institutii rezidentiale, prin furnizarea de servicii sociale si medicale la nivelul comunitatii, inclusiv servicii pe termen lung. Locuintele protejate vor asigura gazduire de tip familial în sistem protejat, îngrijire personală, asistență și suport beneficiarilor, iar în cadrul centrului de zi vor fi organizate activități diverse :informare si consiliere sociala, consiliere psihologica, activitati de abilitare si reabilitare, etc. Beneficiarii plasati la asistent personal profesionist vor beneficia de servicii de locuire, ingrijire personala, hranire cu suport oferit prin Serviciul de asistenta si suport pentru persoanele adulte cu dizabilitati: informare si consiliere sociala, consiliere psihologica, suport individual/grupuri de suport, asistenta si suport pentru luarea unei decizii. Toate aceste activitati se realizeaza în scopul prevenirii reinstituționalizării persoanelor adulte cu dizabilități dezinstitutionalizate prin dobândirea de noi abilități, creșterea stimei de sine și a încrederii în forțele proprii.
