Predmetom projektu je modernizácia zdravotného strediska v Rajeckých Tepliciach, posilnenie všeobecnej ambulantnej zdravotnej starostlivosti a zavedenie poskytovania zdravotnej starostlivosti vo viacerých špecializačných odborochCelkovým cieľom projektu je „Zlepšenie kvality života a zabezpečenie integrovanej a komplexnejšej primárnej zdravotnej starostlivosti pre obyvateľov mesta Rajecké Teplice a jeho spádovej oblasti.“ Celkový cieľ projektu bude dosiahnutý naplnením špecifických cieľov projektu, ktorými sú:modernizácia zdravotného strediska v Rajeckých Tepliciachzlepšenie priestorových podmienok pre poskytovateľov zdravotnej starostlivosti v Rajeckých Tepliciachzvýšenie rozsahu a kapacity poskytovaných služieb zdravotnej starostlivosti obyvateľom mesta a jeho spádovej oblasti Uvedené ciele sa dosiahnu prostredníctvom realizácie dvoch hlavných aktivít projektu:Spracovanie energetického auditu budovy ZSModernizácia a stavebné úpravy budovy zdravotného strediska Rajecké Teplice Miesto realizácie projektu: Rajecké Teplice, Rajecká cesta č, 13., parcely č. 520/1 a 520/2. Výsledkom realizácie projektu bude fungujúce CIZS, spĺňajúce princípy integrovaného modelu starostlivosti o zdravie s dostupnou všeobecnou, špecializovanou i inou špecializovanou ambulantnou starostlivosťou. Realizáciou projektu sa dosiahnu nasledovné merateľné ukazovatele:Počet renovovaných verejných budov - 1Počet vytvorených centier integrovanej zdravotnej starostlivosti - 1Počet zdravotníckych pracovníkov v etablovaných CIZS - 23Podlahová plocha renovovaných verejných budov – 1 508,73 m2Zníženie ročnej spotreby primárnej energie vo verejných budovách - 213 088 kWh/rokOdhadované ročné zníženie emisií skleníkových plynov – 39,097 t ekviv. CO2 Počet nástrojov zabezpečujúcich prístupnosť pre osoby so zdravotným postihnutím - 1Miera zabezpečenia bezbariérového prístupu osôb so zdravotným postihnutím k výsledkom projektu – 100 % (Slovak)
