Objęcie opieką hospicjum domowego 50 osób zagrożonych ubóstwem i wykluczeniem społ. w tym os. niesamodzielnych (26 K 24 M),które chorują na nieuleczalne, postępujące, ograniczające życie choroby nowotworowe i nienowotworowe, o których mowa w zał. nr 1 do rozporządzenia MINISTRA ZDROWIA z dnia 29 października 2013 r. w sprawie świadczeń gwarantowanych z zakresu opieki paliatywnej i hospicyjnej w tym 30% os. niepełnosprawnych do 31.07.2021r. Zaplanowane w proj. zadania przyczynią się do poprawy jakości życia chorych w ostatnim okresie życia (poprzez zapobieganie bólowi i innym objawom somatycznym oraz ich uśmierzanie, łagodzenie cierpień psychicznych, duchowych i socjalnych), a także poprawią jakość życia osób sprawujących opiekę nad nimi, rozumianą jako pomoc w sprawowaniu opieki medycznej i pielęgnacyjnej.
