Projekt "Dzienny Dom Opieki Medycznej w Rudzie Śląskiej" to spójna i kompleksowa forma wsparcia skierowana do 75 pacjentów z terenu Rudy Śląskiej. W ramach projektu będą udzielane świadczenia osobom, wymagającym ze względu na stan zdrowia, świadczeń pielęgnacyjnych, opiekuńczych i rehabilitacyjnych oraz kontynuacji leczenia, a niewymagającym hospitalizacji w oddziale szpitalnym, którzy w ocenie skalą poziomu samodzielności (skala Barthel) otrzymali 40-65 punktów. Projekt zakłada realizację w okresie 01.10.2020 - 31.03.2022 następujących zadań: - Adaptacja infrastruktury Dziennego Domu Opieki Medycznej w w Rudzie Śląskiej na potrzeby funkcjonowania DDOM - Funkcjonowanie Dziennego Domu Opieki Medycznej w Rudzie Śląskiej Grupę docelową stanowią osoby wykluczone lub zagrożone ubóstwem lub wykluczeniem społecznym oraz ich otoczenie z miasta Ruda Śląska. The project will carry out the following tasks in the period 01.10.2020-31.03.2022: — Adaptation of the infrastructure of the Daily Medical Care House in Ruda Śląskia for the purposes of the functioning of the DDOM – Operation of the Daily Medical Care House in Ruda Śląskia The target group consists of persons excluded or at risk of poverty or social exclusion and their surroundings from the city of Ruda Śląska.
