Projekt zapewni opiekę telemedyczną (OT) dla 700 osób K i M (370M i 330 K) zagrożonych wykluczeniem społecznym z woj. śląskiego zamieszkałych na terenie subregionu centralnego. W ramach projektu jego uczestnicy (U) otrzymają usługę zapewniającą OT poleg. na dostępie przez 24 h/dobę 7 dni w tygodniu do konsultacji i porad lekarskich w obszarze kardiologii, geriatrii i ogólnomedycznych a także pakiet konsultacji i porad w zakresie rehabilitacji (w tym ruchowej i kardiologicznej), dietetyki oraz usług pielęgnacyjnych. Dodatkowo proj. zapewni dostępność dla U do lekarza stacjonarnego w razie potrzeby analizy wyników badań lub przeprowadz. dodatkowych badań wymagających udziału osobistego pacjenta. As part of the project, its participants (U) will receive a service providing OT for 24 hours/day 7 days a week for consultations and medical advice in the areas of cardiology, geriatrics and general medical, as well as a package of consultations and advice on rehabilitation (including motor and cardiac), dietetics and care services. In addition, it will provide U with access to a stationary physician if necessary to analyse test results or carry out additional studies requiring the patient’s personal participation. Implementation of the project in the period from 01.01.2020 to 30.06.2021. (English)
