The project is aimed at 500 people (260K,240M), requiring comprehensive rehabilitation, active occupation or declaring readiness to work, with chronic diseases of bone-and-articular punctures and muscles from the Cieszyn district. The aim of the project is to increase the possibility of benefiting from rehab., health services aimed at improving the functioning of their health status and support for social and occupational activation. The project presupposes the implementation of an Individual Rehabilitation Plan and the education of patients by qualified staff, which will meet the criteria set out in the Polish Republic of Poland. It is also intended to support actions to increase the availability to a comprehensive therapeutic rehab (including carriage) and to increase the level of knowledge of the participants/cheques of the project in the field of secondary prevention of chronic bone-articular and muscle diseases. Projekt skierowany jest do 500 osób (260K,240M), wymagających kompleksowej rehabilitacji, aktywnych zawod. lub deklarującym gotowość do podjęcia pracy, z przewlekłymi chorobami ukł. kostno-staw. i mięśniow. z terenu powiatu cieszyńskiego. Celem proj. jest zwiększenie możliwości skorzystania ze świadczeń rehab., zdrowotnych ukierunkowanych na poprawę funkcjonowania ich stanu zdrowia oraz wsparcie w zakresie aktywizacji społecznej i zawodowej. Proj. zakłada przeprowadzenie Indywidualnego Planu Rehabilitacyjnego oraz edukację pacjentów prowadzoną przez wykwalifikowaną kadrę, która będzie spełniać kryteria określone w RPZ. Zakłada się również wsparcie działań na rzecz zwiększenia dostępności do kompleksowej rehab. leczniczej (m.in.dowóz) oraz podniesienia poziomu wiedzy uczestników/czek proj. z zakresu profilaktyki wtórnej chorób przewlekłych ukł. kostno-stawowego i mięśniowego. Dla personelu med.(25os.) zaangażowanego we wsparcie pacjentów beneficjent przewiduje specjalistyczne szkolenia. (Polish)
