The project was based on the results of an individual local diagnosis of the need for social services (caring and specialised care), carried out by PSONI in Gdynia in cooperation with the City of Gdynia and its subordinate institutions, including in particular the MOPS and the Independent Referral for Persons with Disabilities. The project aims to support people with disabilities up to 25 years and their actual guardians. As part of the project, 30 places of care will be created in the newly established day care centre (hereinafter DOOW) at PSONI in Gdynia, under the name of the Day Rest Care and Social Rehabilitation Centre “Young Estate”, for 240 (120K, 120M) underprivileges aged 0-25, who, due to their age, have specific and diverse needs related to part-progress, which we will take into account in individual reintegration pathways. 190 (170K, 20M) guardians of the actual participants in the care of the centre will also receive support. The project fits into the first type of projects. The project will be implemented in the sense that respite care can not only be a “storage room”, but should provide learning activities of everyday life, support for self-reliance, teach separation from the parent in a safe environment. Our project fits into the law “for life” and activities in the context of respite care. Therefore, a partnership has been concluded for the project, in which cooperation will be carried out with the municipality of Gdynia, MOPS and NRdSON in order to recruit, develop individual pathways for the reintegration of project participants and ensure high quality of activities at each stage of the project. The guarantee of high efficiency of the project is the fact that PSONI, as part of its current activity for disabled people, has been cooperating with the above-mentioned institutions for years, and this cooperation is based on mutual trust and is appreciated by the City of Gdynia. On the basis of partnership, including with other actors, we also want to base the continuation of the project after the end of its ESF funding. (English) | |||||||||||||||

Projekt powstał w oparciu o wyniki indywidualnej lokalnej diagnozy zapotrzebowania na usługi społeczne (opiekuńcze i specjalistyczne opiekuńcze), przeprowadzonej przez PSONI w Gdyni we współpracy z Miastem Gdynia i podległymi mu placówkami, w tym w szczególności z MOPS oraz Niezależnym Referatem do Spraw Osób Niepełnosprawnych. Projekt ma na celu wsparcie niepełnosprawnych osób do 25r.ż. i ich opiekunów faktycznych. W ramach proj. powstanie 30 miejsc opieki w nowopowstałym dziennym ośrodku opieki wytchnieniowej (dalej DOOW) przy PSONI w Gdyni, pod nazwą Dzienny Ośrodek Opieki Wytchnieniowej i Rehabilitacji Społecznej "Osiedle Młodych", dla 240 (120K, 120M) niepełnospr. podopiecznych w wieku 0-25 lat, którzy ze względu na wiek mają specyficzne i zróżnicowane potrzeby związane z niepełnospr., co uwzględnimy w indywidualnych ścieżkach reintegracji. Wsparcie otrzyma również 190 (170K, 20M) opiekunów faktycznych uczestników objętych opieką ośrodka. Projekt wpisuje się w I typ projektów. Projektu realizowany będzie w myśl, że opieka wytchnieniowa nie może być tylko „przechowalnią”, ale powinna zapewniać naukę czynności życia codziennego, wspieranie ku samodzielności, uczyć separacji z rodzicem w bezpiecznym otoczeniu. Nasz projekt wpisuje się w ustawę „za życiem” i działania w ramach opieki wytchnieniowej. W związku z tym na rzecz projektu zawarte zostało partnerstwo, w ramach którego będzie realizowana współpraca z gminą Gdynia, MOPS i NRdSON w celu rekrutacji, opracowania indywidualnych ścieżek reintegracji uczestników proj. i zapewni wysoką jakość działań na każdym etapie proj. Gwarantem wysokiej efektywności proj. jest fakt, że PSONI w ramach bieżącej działalności na rzecz os. niepełnosprawnych od lat współpracuje z w/w placówkami, a współpraca ta jest oparta na wzajemnym zaufaniu i jest doceniana przez Miasto Gdynia. Na zasadzie partnerskiej współpracy, także z innymi podmiotami, chcemy również opierać kontynuację projektu po zakończeniu jego finansowania z EFS. (Polish)
