The project was implemented in the period 01.06.2020-30.09.2021 by “REMED” RENATA Bienias in partnership with Majchrzyk Piotr TRANS-MEDYK RATOWNICTWO.The project implements the following types of project:1)Development of deinstitutionalised forms of medical care for the elderly, including with disabilities, in the field of support for activities or the creation of new medical care facilities, including long-term home care, providing medical care in lieu of de facto carers, increasing access to services by providing the necessary transport and improving the skills and qualifications of staff of services providing health services concerning deinstitutionalised forms; 2)Development of activities that improve the health of older people, including those with disabilities through motor rehabilitation, psychochiatric rehabilitation; psychological support and/or training for actual caregivers in the field of medical care. The project is aimed at people or families at risk of poverty or social exclusion, around people at risk of poverty or social exclusion and staff of health services in the voivodship. The support will be extended to 8 patients (6K and 2M), 5(4K;1M) personnel of the service providing zdrowotne:pielęgniarki(1K;0M),medical caregivers/nurses(1K,0M),psychologist(1K,0M),doctor-psychiatra/geriatra (1K,1M).This project will ensure the provision of health services for the elderly, including those with disabilities. Projekt realizowany w okresie 01.06.2020-30.09.2021 przez "REMED" RENATA BIENIAS w partnerstwie z Majchrzyk Piotr TRANS-MEDYK RATOWNICTWO.Projekt realizuje następujące typy projektu:1)Rozwój zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami, w zakresie m.in.:wsparcia działalności lub tworzenia nowych miejsc opieki medycznej, w tym długoterminowej medycznej opieki domowej,zapewnienia opieki medycznej w zastępstwie za opiekunów faktycznych,zwiększenia dostępu do usług poprzez zapewnienie niezbędnego transportu oraz podnoszenia kompetencji i kwalifikacji personelu służb świadczących usługi zdrowotne dotyczące form zdeinstytucjonalizowanych; 2)Rozwój działań wpływających na poprawę stanu zdrowia osób starszych, w tym z niepełnosprawnościami poprzez rehabilitację ruchowa,psychiatryczną;wsparcie psychologiczne i/lub szkolenia dla opiekunów faktycznych w zakresie opieki medycznej.Projekt skierowany jest do osób lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym,otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym oraz personelu służb świadczących usługi zdrowotne zamieszkujących teren woj.opolskiego.Wsparciem zostanie objętych 8 pacjentów(6K i 2M), 5(4K;1M) osób stanowiących personel służb świadczących usługi zdrowotne:pielęgniarki(1K;0M),opiekunowie medyczni/pielęgniarki(1K,0M),psycholog(1K,0M),lekarz-psychiatra/geriatra (1K,1M).W ramach niniejszego projektu zostanie zapewnione świadczenie usług zdrowotnych nad osobami starszymi, w tym z niepełnosprawnością.
