MAIN OBJECTIVE: The main objective of the project is to increase social and labour market participation and improve access to the labour market of 32 people (20K and 12M), excluded (including those affected by poverty) or at risk of poverty or social exclusion, benefiting from GOPS support from the municipality of Stary Zamość through a comprehensive support programme including the creation of an individual reintegration pathway, as well as support with the use of active inclusion services of a social and/or occupational and/or educational and/or health nature within 24 months of the project’s implementation. MAIN RESULTS: Number of people at risk of poverty or social exclusion who obtained a qualification or competence after leaving the programme – 22 persons, Number of people at risk of poverty or social exclusion working after leaving the programme (including the self-employed) – 4 persons, Number of people at risk of poverty or social exclusion looking for a job after leaving – 3 persons. TARGET GROUPS: The target group shall be excluded persons (including those affected by poverty) or persons at risk of poverty or social exclusion from the municipality of Stary Zamość over 18 years old who are customers of GOPS – 32 people (20K and 12M), including in particular: inactive persons (b) unemployed persons at risk of exclusion on grounds other than unemployment, for whom only labour market instruments and services are insufficient and there is a need to prioritise active inclusion services with a social dimension of a total of 32 persons and 38 people in the vicinity of people at risk of social exclusion. The project will also support the environment of socially excluded people, i.e. families of people who are excluded or at risk of poverty and social exclusion (spouses and children), whose participation in the project is necessary for the effective support of project participants. CEL GŁÓWNY: Celem głównym projektu jest wzrost aktywności społecznej i zawodowej oraz poprawa dostępu do rynku pracy 32 osób (20K i 12M), wykluczonych (w tym dotkniętych ubóstwem) lub zagrożonych ubóstwem lub wykluczeniem społecznym, korzystających ze wsparcia GOPS z terenu gminy Stary Zamość poprzez kompleksowy program wsparcia obejmujący stworzenie indywidualnej ścieżki reintegracji a także wsparcie z wykorzyatniem usług aktywnej integracji o charakterze społecznym i/lub zawodowym i/lub edukacyjnym i/lub zdrowotnym w okresie 24 miesięcy realizacji projektu. GŁÓWNE REZULTATY: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje lub kompetencje po opuszczeniu programu - 22 osoby, Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) - 4 os, Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu - 3 os. GRUPY DOCELOWE: Grupę docelową stanowią osoby wykluczone(w tym dotknięte ubóstwem) lub osoby zagrożone ubóstwem lub wykluczeniem społecznym powyżej 18 r.ż. z terenu Gminy Stary Zamość będące klientami GOPS - 32 osoby (20K i 12M), w tym w szczególności: a) osoby bierne zawodowo b) osoby bezrobotne-zagrożone wykluczeniem ze względu na przesłanki inne niż tylko bezrobocie, wobec których zastosowanie wyłącznie instrumentów i usług rynku pracy jest niewystarczające i istnieje konieczność zastosowania w pierwszej kolejności usług aktywnej integracji o charakterze społecznym Łącznie 32 osoby i 38 osób z otoczenia osób zagrożónych wykluczeniem społecznym. Wsparciem w ramach projektu zostanie objęte również otoczenie osób wykluczonych społecznie tj. rodziny osób wykluczonych lub zagrożonych ubóstwem i wykluczeniem społecznym (małżonkowie i dzieci), których udział w projekcie jest niezbędny dla skutecznego wsparcia uczestników projektu.
